• B Wilke Construction LLC, Omro, must pay a $3,000 ladders citation, a $6,000 eye-and-face-protection citation and an $8,400 fall-protection citation because the period to contest the violations with a Wisconsin OSHA office has ended. The company was issued the citations June 12 during a roofing job at 1201 Michigan Ave., Oshkosh.

• A-Plus Roofing LLC, Bonduel, paid a $2,800 fall-protection citation July 5. The company was issued the citation June 8 during a roofing job in Luxemburg.

• T. Steele Construction Inc., Rock Island, Ill., reached an informal settlement with OSHA on Aug. 6 to reduce a $2,000 fall-protection citation to $1,400. The company was issued the citation July 12 during a heavy and civil engineering job at 605 N. Eighth St., Sheboygan.

• Kaltenbrun Brothers Roofing Co. Inc., Sheboygan, reached an informal settlement with OSHA on Aug. 6 to reduce a $1,200 fall-protection citation to $840. The company was issued the citation July 13 during a roofing job at 531 Giddings Ave., Sheboygan Falls.

• Quest Building Corp., Waunakee, reached an informal settlement with OSHA on Aug. 7 to reduce a $10,780 fall-protection citation to $3,460. The company was issued the citation July 23 during a single-family housing job at 427 S. Bristol St., Sun Prairie.

• Velcheck & Finger Roof Consulting & Service Inc., Pewaukee, reached an informal settlement with OSHA on Aug. 7 to reduce three fall-protection citations, totaling $7,854, to $1,500. The company was issued the citations July 25 during a roofing job at N168 W21367 Main St., Jackson.

• TK Woods Unlimited Inc., De Pere, reached an informal settlement with OSHA on Aug. 8 to reduce a $2,800 fall-protection citation to $1,600. The company was issued the citation July 13 during a framing job at 690 Casaloma Drive, Grand Chute.

• Exterior Designs LLC, Little Suamico, reached an informal settlement with OSHA on Aug. 9 to reduce a $1,020 fall-protection citation to $714. The company was issued the citation July 26 during a roofing job at 1134 Guns Road, Green Bay.

• Pro-Type Builders Inc., Dalton, reached an informal settlement with OSHA on Aug. 10 to reduce a $4,620 rough-terrain-forklift citation to $2,500, a $4,620 material-handling-equipment citation to $0, a $3,300 rigging-equipment-for-material-handling citation to $2,000, a $4,620 cranes-and-derricks-inspections citation to $2,500 and a $9,240 steel-erection-fall-protection citation to $2,500. The company and OSHA also agreed to dismiss a $4,620 cranes-and-derricks-keeping-clear-of-the-load citation. The company was issued the citations July 18 during a structural steel and precast concrete job at 450 Ripple Road, Oshkosh.

• Jose Hernandez Home Repair, Carpentersville, Ill., reached an informal settlement with OSHA on Aug. 13 to reduce a $2,400 fall-protection citation to $1,680 and a $2,400 fall-protection-training citation to $1,680. The company was issued the citations Aug. 3 during a roofing job at 1106 Center Ave., Janesville.

• Superior Exteriors Inc., Brookfield, reached an informal settlement with OSHA on Aug. 13 to reduce a $2,800 fall-protection citation to $1,960. The company was issued the citation Aug. 3 during a roofing job at 2510 Plaza Court, Waukesha.

• Gerard Chimney Co., St. Louis, reached a formal settlement with OSHA on Aug. 13 to reduce a $5,390 scaffolding citation to $2,500 and a $3,272 ladders citation to $1,000. The company and OSHA also agreed to dismiss a $3,850 fall-protection-training citation. The company was issued the citations Aug. 9, 2011, during a wrecking and demolition job at 913 57th St., Kenosha.

• Long Life Inc., Wisconsin Dells, reached an informal settlement with OSHA on Aug. 13 to reduce a $6,160 fall-protection citation to $2,500. The company was issued the citation July 20 during a roofing job at 1305 Kalahari Way, Wisconsin Dells. Long Life also reached an informal settlement Aug. 13 to reduce a $2,000 fall-protection citation to $1,000. The company was issued the citation July 20 during a roofing job at 1108 Superior Ave., Tomah.

• DH Construction Inc., Evansville, reached an informal settlement with OSHA on Aug. 15 to reduce a $4,200 fall-protection citation to $2,400. The company was issued the citation July 23 during a roofing job at 803 Rockport Road, Janesville.

• Absolute Home Improvements Inc., South Milwaukee, reached an informal settlement with OSHA on Aug. 16 to reduce a $2,800 fall-protection citation to $1,500 and a $2,800 ladders citation to $500. The company was issued the citations July 27 during a roofing job at 9408 W. Marion St., Wauwatosa.
