OSHA reports • Affordable Quality and Excellence, Wauwatosa, reached a formal settlement with OSHA on March 30 to pay a $2,000 scaffolding-training citation, a $1,400 general-safety-and-health-provisions citation and 10 scaffolding citations, totaling $17,000.
