  • Hammer On Custom Carpentry, Neenah, must pay a $1,050 fall-protection citation because the period to contest the violation has ended. The company was issued the citation Dec. 2 during a roofing job in Greenville.
  • Travis Markiewicz Construction LLC, Oconto Falls, must pay a $1,050 eye-and-face-protection citation and a $750 electrical-wiring-design-and-protection citation because the period to contest the violations has ended. The company was issued the citations Feb. 10 during a framing job in Green Bay.
  • Kavid Exteriors LLC, Madison, must pay a $1,500 fall-protection citation because the period to contest the violation has ended. The company was issued the citation Feb. 24 during a roofing job at 910 N. 10th St., Sheboygan.
  • Martin Construction Inc., Mount Horeb, must pay a $1,050 scaffolding citation, a $1,050 fall-protection citation and a $1,050 power-transmission-tools-and-protective-equipment citation because the period to contest the violations has ended. The company was issued the citations April 24, 2008, during a roofing job at 2574 Honey Creek Circle, East Troy.
  • Ace Plastering, Beaver Dam, reached an informal settlement with OSHA on June 19, 2008, to reduce a $1,050 scaffolding-training citation to $735 and two scaffolding citations, totaling $2,100, to $1,470. The company was issued the citations June 3, 2008, during a drywall and insulation job in Madison.
  • Allen Howe & Son Inc., Waterloo, reached a formal settlement with OSHA on July 7, 2008, to reduce two specific-excavation-requirements citations, totaling $23,050, to $5,500 and two requirements-for-excavation-protective-systems citations, totaling $30,100, to $9,500. The company was issued the citations Dec. 4, 2007, during a water- and sewer-line job at 5005 Card Ave., McFarland.
  • Quality Home Improvement, Rudolph, reached an informal settlement with OSHA on July 31, 2008, to reduce a $4,000 fall-protection citation to $1,600. The company was issued the citation July 8, 2008, during a roofing job at 1540 Woodvine St., Wisconsin Rapids.
  • Affordable Quality and Excellence, Wauwatosa, reached a formal settlement with OSHA on March 30 to pay a $2,000 scaffolding-training citation, a $1,400 general-safety-and-health-provisions citation and 10 scaffolding citations, totaling $17,000. The company was issued the citations July 17, 2008, during a masonry job at 518 W. Cherry St., Milwaukee.
