- Kavid Exteriors LLC, Madison, must pay a $1,500 fall-protection citation because the period to contest the violation has ended. The company was issued the citation Feb. 24 during a roofing job at 910 N. 10th St., Sheboygan.

- Quality Home Improvement, Rudolph, reached an informal settlement with OSHA on July 31, 2008, to reduce a $4,000 fall-protection citation to $1,600. The company was issued the citation July 8, 2008, during a roofing job at 1540 Woodvine St., Wisconsin Rapids.
- Affordable Quality and Excellence, Wauwatosa, reached a formal settlement with OSHA on March 30 to pay a $2,000 scaffolding-training citation, a $1,400 general-safety-and-health-provisions citation and 10 scaffolding citations, totaling $17,000. The company was issued the citations July 17, 2008, during a masonry job at 518 W. Cherry St., Milwaukee.
