  • Affordable Quality and Excellence, Wauwatosa, reached a formal settlement with OSHA on March 30 to pay a $2,000 scaffolding-training citation, a $1,400 general-safety-and-health-provisions citation and 10 scaffolding citations, totaling $17,000. The company was issued the citations July 17, 2008, during a masonry job at 518 W. Cherry St., Milwaukee.
  • Schmidt Roofing of Kewaskum, WI. Inc., Allenton, must pay a $2,800 fall-protection citation, a $1,000 ladders citation, a $1,400 fall-protection-training citation and two scaffolding citations, totaling $2,800 because the period to contest the violations has ended. The company was issued the citations Oct. 21, 2008, during a roofing job at 328 E. Washington Drive, Kewaskum.
  • Siding Brothers, Racine, must pay a $750 scaffolding citation and a $1,050 ladders citation because the period to contest the violations has ended. The company was issued the citations Oct. 28, 2008, during a siding job in New Berlin.
  • Custom Exteriors LLC, Muskego, must pay a $375 fall-protection citation because the period to contest the violation has ended. The company was issued the citation Oct. 9, 2008, during a finishing job at 2651 Wauwatosa Ave., Wauwatosa.
  • Milwaukee Home Improvements, Milwaukee, must pay a $450 criteria-for-personal-protective-equipment citation, a $600 fall-protection citation and a $600 fall-protection-training citation because the period to contest the violations has ended. The company was issued the citations Sept. 25, 2008, during a roofing job at 4620 N. Morris Blvd., Whitefish Bay.

  • VMA Enterprises LLC, Cleveland, Tenn., must pay three scaffolding citations, totaling $3,750, because the period to contest the violations has ended. The company was issued the citations Oct. 24, 2008, during a masonry job at 1500 Horizon Blvd., Sturtevant.
  • Murdock Construction LLC, Kenosha, must pay a $600 fall-protection citation because the period to contest the violation has ended. The company was issued the citation Oct. 23, 2008, during a framing job at 1627 30th Court, Kenosha.
  • Dane Construction LLC, Oshkosh, reached a formal settlement with OSHA on April 17 to reduce a $10,000 fall-protection citation to $1,000. The company was issued the citation Nov. 12, 2008, during a roofing job at 685 Watermark Court, Appleton. Dane also reached a formal settlement with OSHA on April 17 to reduce a $1,000 ladders citation to $100 and a $10,000 fall-protection citation to $1,000. The company was issued those citations Nov. 12, 2008, during a roofing job in Appleton.
  • M&M Mechanical LLC, Mukwonago, reached a formal settlement with OSHA on April 20 to reduce a $1,050 access-to-employee-exposure-and-medical-records citation to $375 and seven lead citations, totaling $7,350, to $2,625. The company was issued the citations July 16, 2008, during a remediation job at 540 S. First St., Milwaukee.
  • Merced Mascotes Construction, Juneau, reached an informal settlement with OSHA on April 20 to reduce a $1,500 fall-protection citation to $900 and a $1,500 fall-protection-training citation to $900. The company was issued the citations April 15 during a roofing job at 304 E. Maple Ave., Beaver Dam.
