The company was issued the citations July 17, 2008, during a masonry job at 518 W. Cherry St., Milwaukee.
• LK Construction, Milwaukee, must pay a $750 eye-and-face-protection citation, a $750 powered-industrial-trucks citation and a $750 scaffolding citation because the period to contest the violations has ended. The company was issued the citations Oct. 8, 2008, during a wrecking and demolition job at 19555 W. Bluemound Road, Brookfield.
• Iverson Design Services Inc., Harrison, Ark., must pay a $750 self-propelled-work-platform citation; two ladders citations, totaling $2,100; and two stairways-and-ladders-training citations, totaling $2,100, because the period to contest the violations has ended. The company was issued the citations Oct. 24, 2008, during a framing job in Brookfield.
• Foremost Decorating Inc., Racine, must pay a $1,500 aerial-lifts citation and a $1,500 fall-protection-training citation because the period to contest the violations has ended. The company was issued the citations Oct. 6, 2008, during a painting and wall-covering job at 1500 92nd St., Sturtevant.

• Surges Electric Inc., Franklin, reached an informal settlement with OSHA on May 8 to reduce a $1,500 fire-protection citation to $1,050; a $525 stairways citation to $367.50; two electrical-installation-safety-requirements citations, totaling $675, to $472.50; and three electrical-wiring-methods citations, totaling $1,050, to $735. The company was issued the citations May 5 during an electrical job at 16415 Wisconsin Ave., Brookfield.
• Alpine Plumbing Inc., Brookfield, reached an informal settlement with OSHA on May 11 to reduce a $2,500 fire-protection citation to $1,750 and a $1,225 stairways citation to $857.50. The company was issued the citations May 5 during a plumbing, heating and air-conditioning job at 16415 Wisconsin Ave., Brookfield.
• Highland Construction Inc., Hartford, reached an informal settlement with OSHA on May 20 to reduce a $1,500 fire-protection citation to $400, a $1,050 electrical-wiring-methods citation to $400 and a $1,050 stairways citation to $400. • Homburg Contractors Inc., Monona, reached an informal settlement with OSHA on May 20 to reduce a $2,500 requirements-for-excavation-protective-systems citation to $1,750. The company was issued the citation April 29 during an excavation job in Madison.
• Bob Anderson Builders Inc., Waukesha, reached an informal settlement with OSHA on May 20 to reduce a $1,500 aerial-lifts citation to $800. The company was issued the citation April 29 during a framing job at 1306 W. Wisconsin Ave., Oconomowoc.
• C.G. Enterprises Inc., South Milwaukee, reached an informal settlement with OSHA on May 21 to reduce a $1,500 mobile-crane citation to $750, a $1,500 below-the-hook-lifting-device citation to $750 and a $1,500 crawler-locomotive-and-truck-cranes citation to $0. The company and OSHA also agreed to dismiss a $1,500 crawler-locomotive-and-truck-cranes citation and a $1,500 mobile-crane citation. The company was issued the citations May 4 during a specialty-trade job at 1919 S. Marina Drive, Milwaukee.
• Tesmer Construction Corp., Neillsville, reached an informal settlement with OSHA on May 26 to reduce a $1,000 demolition-preparatory-operations citation to $500 and a $2,000 reporting-fatalities-and-multiple-hospitalization-incidents-to-OSHA citation to $1,000. The company also agreed to pay two fall-protection citations, totaling $4,000. The company was issued the citations May 6 during a wrecking and demolition job in Galloway.
• Quality Roofing Inc., Marshfield, reached a formal settlement with OSHA on May 27 to reduce a $5,600 safety-training-and-education citation to $3,000. The company was issued the citation Oct. 23, 2008, during a roofing job at 7800 International Drive, Wausau.
• B&B Sheet Metal Inc., La Crosse, reached an informal settlement with OSHA on May 28 to pay a $300 electrical-wiring-design-and-protection citation and to dismiss a $600 electrical-wiring-design-and-protection citation and an $800 electrical-wiring-methods citation. The company was issued the citations May 27 during a finishing job at 175 Sumner St., La Crosse.
• Donovan & Jorgenson Inc., New Berlin, reached an informal settlement with OSHA on May 28 to reduce a $2,500 fire-protection citation to $1,250, an $875 electrical-wiring-methods citation to $437.50 and a $1,225 stairways citation to $612.50. The company was issued the citations May 5 during a specialty-trade job at 16415 Wisconsin Ave., Brookfield.
