•    Leach Brothers Construction LLC, Oshkosh, reached an informal settlement with OSHA on July 1 to reduce a $1,500 steel-erection-fall-protection citation to $1,050. The company was issued the citation June 24 during a nonresidential building job in Appleton.
•    Long Life Roofing Inc., Wisconsin Dells, reached an informal settlement with OSHA on July 2 to reduce a $600 fall-protection citation to $200. The company was issued the citation June 19 during an industrial building job in Wisconsin Dells.
•    Sawfish Construction LLC, Milwaukee, reached an informal settlement with OSHA on July 7 to reduce a $1,500 scaffolding-training citation to $0 and four scaffolding citations, totaling $6,000, to $3,000. The company and OSHA also agreed to dismiss a $1,500 scaffolding-training citation and three scaffolding citations, totaling $4,500. The company was issued the citations June 17 during a residential remodeling job at 3017 S. Superior St., Milwaukee.
•    Badger Roofing of Wisconsin, Wausau, reached an informal settlement with OSHA on July 10 to reduce a $1,050 fall-protection citation to $735. The company was issued the citation June 29 during a roofing job at 321 S. 52nd Ave., Wausau.
•    All State Roofing Inc., Madison, reached an informal settlement with OSHA on July 14 to reduce a $1,000 fall-protection citation to $500. The company was issued the citation June 30 during a nonresidential building job at 640 Enterprise Drive, Verona.
•    CJC Construction LLC, Green Bay, reached a formal settlement with OSHA on July 16 to reduce a $600 eye-and-face-protection citation to $420 and a $1,050 scaffolding citation to $735. The company was issued the citations June 18 during a single-family housing job at 999 Astor Court, Ashwaubenon.
•    Steffens Roofing Inc., Oconto Falls, reached informal settlements with OSHA on July 19 and July 27 to reduce a $2,800 fall-protection citation to $1,400 and a $1,600 eye-and-face-protection citation to $800. The company was issued the citations July 9 during a roofing job at 967 Springfield Drive, De Pere.
•    Adams & Sons Ltd., Janesville, reached an informal settlement with OSHA on July 21 to reduce a $3,000 fall-protection citation to $1,500. The company was issued the citation July 8 during a roofing job at 256 W. Main St., Evansville.
•    Innovative Construction LLC, De Pere, reached an informal settlement with OSHA on July 22 to increase a $600 aerial-lifts citation to $1,050 and to pay a $450 eye-and-face-protection citation. The company was issued the citations July 10 during an industrial building job at 2475 S. Oneida St., Green Bay.
•    Advanced Seamless Gutters Plus Inc., Evansville, reached an informal settlement with OSHA on July 23 to reduce a $2,800 fall-protection citation to $1,120 and a $2,800 fall-protection-training citation to $1,120. The company was issued the citations July 8 during a finishing job in Evansville.
•    Milakovich Roof Maintenance Inc., De Pere, reached an informal settlement with OSHA on July 28 to reduce a $600 eye-and-face-protection citation to $300 and a $1,050 fall-protection citation to $600. The company was issued the citations July 9 during a roofing job in Ashwaubenon.
•    Central Wisconsin Roofing Inc., Schofield, reached an informal settlement with OSHA on Aug. 4 to reduce a $1,000 fall-protection citation to $350. The company was issued the citation July 31 during a roofing job in Appleton.
•    Southport Engineered Systems LLC, Kenosha, reached an informal settlement with OSHA on Aug. 4 to reduce a $3,000 fall-protection citation to $1,200. The company was issued the citation July 22 during a plumbing, heating and air-conditioning job at 8853 S. Howell Ave., Oak Creek.
•    Walsh Masonry Inc., Ixonia, reached an informal settlement with OSHA on Aug. 4 to reduce a $2,500 fall-protection citation to $1,500. The company was issued the citation July 16 during a masonry job at 420 N. Park St., Madison.
•    Walsh Masonry Inc., Ixonia, reached an informal settlement with OSHA on Aug. 5 to reduce two scaffolding citations, totaling $1,400, to $550. The company was issued the citations July 17 during a masonry job at 34700 Valley Road, Oconomowoc.
•    Allen Steele Co. Inc., Lake Delton, reached an informal settlement with OSHA on Aug. 5 to reduce a $2,500 specific-excavation-requirements citation to $0 and a $2,500 requirements-for-protective-systems citation $2,000. The company was issued the citations July 28 during an excavation job in Reedsburg.
•    American RSI, Beaver Dam, reached an informal settlement with OSHA on Aug. 6 to pay a $2,000 fall-protection citation and reduce an $800 head-protection citation to $500, a $2,800 fall-protection-training citation to $1,000, a $2,000 ladders citation to $1,300 and a $1,000 stairways-and-ladders-training citation to $0. The company and OSHA also agreed to dismiss a $1,000 scaffolding citation. The company was issued the citations July 15 during a roofing job in Juneau.
•    Magill Construction Co. Inc., Elkhorn, reached an informal settlement with OSHA on Aug. 11 to reduce a $1,500 general-safety-and-health-provisions citation to $300. The company was issued the citation July 22 during a nonresidential building job at 8853 S. Howell Ave., Oak Creek. Magill also reached an informal settlement with OSHA on Aug. 11 to pay a $1,500 fall-protection citation and a $1,500 fall-protection-training citation. The company was issued the citations July 24 during a nonresidential building job at 8288 Antioch Road, Salem.
•    Francis Ziegler Builders LLC, Waunakee, reached an informal settlement with OSHA on Aug. 11 to reduce a $600 powered-industrial-trucks citation to $210 and three scaffolding citations, totaling $4,500, to $1,575. The company was issued the citations July 16 during a siding job in Madison.
•    Reedy Industries Inc., Kenosha, reached an informal settlement with OSHA on Aug. 11 to reduce a $2,500 fall-protection citation to $1,250. The company was issued the citation July 21 during a plumbing, heating and air-conditioning job at 9916 75th St., Kenosha.
•    Kenosha Plumbing Inc., Kenosha, reached an informal settlement with OSHA on Aug. 12 to reduce a $300 recording-criteria-for-needlestick-and-sharps-injuries citation to $100 and two hazard-communication citations, totaling $900, to $450. The company and OSHA also agreed to dismiss two hazard-communication citations, totaling $900. The company was issued the citations July 27 during a plumbing, heating and air-conditioning job at 561 11th Place, Kenosha.
•    Keith Lenss Construction, Green Bay, reached an informal settlement with OSHA on Aug. 13 to reduce a $600 fall-protection citation to $240. The company was issued the citation Aug. 5 during a single-family housing job at 1735 Oneida St., Green Bay.
•    United States Fire Protection Inc., Lake Forest, Ill., reached a formal settlement with OSHA on Aug. 13 to reduce a $1,925 stairways citation to $1,200 and to dismiss a $1,925 fire-protection citation. The company was issued the citations April 3 during a specialty-trade job at 16415 Wisconsin Ave., Brookfield.
•    Cablecom LLC, Milwaukee, reached an informal settlement with OSHA on Aug. 13 to reduce a $2,500 requirements-for-excavation-protective-systems citation to $1,800 and four specific-excavation-requirements citations, totaling $10,000, to $2,800. The company and OSHA also agreed to dismiss two specific-excavation-requirements citations, totaling $5,000. The company was issued the citations July 28 during a specialty-trade job in Kenosha.
•    Leach Brothers Construction LLC, Oshkosh, reached an informal settlement with OSHA on Aug. 14 to reduce an $800 fall-protection citation to $295. The company was issued the citation Aug. 14 during a commercial and institutional building job in Appleton.
•    Dane County Contracting LLC, Dane, reached an informal settlement with OSHA on Aug. 14 to reduce a $1,500 requirements-for-excavation-protective-systems citation to $1,050. The company was issued the citation July 29 during a water- and sewer-line job in Waunakee.
•    Kjelstead Plumbing & Heating Inc., Eau Claire, paid a $600 general-safety-and-health-provisions citation and a $2,000 specific-excavation-requirements citation Aug. 14. The company was issued the citations June 11 during a plumbing, heating and air-conditioning job at 3217 Stein Blvd., Eau Claire.
•    Complete Painting LLC, Milwaukee, reached a formal settlement with OSHA on Aug. 14 to reduce a $1,500 fall-protection citation to $50; a $1,500 fall-protection-training citation to $50; three hazard-communication citations, totaling $2,400, to $150; and six lead citations, totaling $8,250, to $300. The company was issued the citations Dec. 18, 2008, during a painting and wall-covering job at 2718 N. Downer Ave., Milwaukee.
•    Ken Kabanuk, Harris, Minn., reached an informal settlement with OSHA on Aug. 18 to reduce a $1,500 fall-protection citation to $900 and a $1,500 ladders citation to $900. The company was issued the citations July 22 during a single-family housing job at 603 Main St., Eau Claire.
•    Bridges R Us Painting Co. Inc., Campbell, Ohio, paid a $4,200 lead citation and a $1,500 aerial-lifts citation on Aug. 18. The company was issued the citations July 15 during a specialty-trade job in La Crosse.
•    Donovan & Jorgenson Inc., New Berlin, reached an informal settlement with OSHA on Aug. 19 to pay a $1,125 fall-protection citation and to reduce a $1,125 ladders citation to $0. The company was issued the citations July 28 during a plumbing, heating and air-conditioning job at 1125 James Drive, Hartland.
•    Matt Griswold Masonry, Oconomowoc, reached an informal settlement with OSHA on Aug. 20 to reduce a $1,500 scaffolding citation to $300. The company was issued the citation July 28 during a masonry job at 158 Wisconsin Ave., Oconomowoc.
•    BPX Contractors Inc., Pewaukee, reached an informal settlement with OSHA on Aug. 20 to reduce a $1,500 hydraulic-excavator citation to $750. The company was issued the citation July 30 during an excavation job in North Prairie.
•    Diaz Roofing, Edgerton, reached an informal settlement with OSHA on Aug. 20 to reduce a $1,000 head-protection citation to $400, a $1,000 ladders citation to $400, a $2,800 eye-and-face-protection citation to $1,120 and a $4,000 fall-protection citation to $1,600. The company was issued the citations Aug. 18 during a roofing job at 109 Main St., Monticello.
•    Camosy Inc., Kenosha, reached an informal settlement with OSHA on Aug. 24 to reduce a $2,500 scaffolding citation to $1,250. The company was issued the citation July 30 during a nonresidential building job at 7101 53rd St., Kenosha.
•    Pro-Type Builders Inc., Dalton, reached an informal settlement with OSHA on Aug. 24 to reduce a $625 fall-protection citation to $437. The company was issued the citation Aug. 7 during an industrial building job at 1516 Fernando Drive, Ashwaubenon.
•    Lehman Masonry Inc., Portage, reached an informal settlement with OSHA on Aug. 26 to reduce a $1,000 powered-industrial-trucks citation to $500 and a $2,000 requirements-for-masonry-construction citation to $1,000. The company was issued the citations Aug. 19 during a masonry job at 142 E. Tyranena Park Road, Lake Mills.
•    Cruz’s Roofing & Construction of Madison Inc., Madison, reached an informal settlement with OSHA on Aug. 27 to reduce a $600 head-protection citation to $100, a $4,000 fall-protection citation to $2,500 and a $4,000 fall-protection-training citation to $2,500. The company was issued the citations July 28 during a roofing job at 3114 Oakridge Road, Madison.
•    Suburban Electrical Engineers/Contractors Inc., Appleton, reached an informal settlement with OSHA on Aug. 31 to pay a $700 safety-related-work-practices citation. The company was issued the citations Aug. 13 during an electrical job at 4670 Integrity Way, Appleton.
