•    Design Structures LLC, Middleton, reached an informal settlement with OSHA on Nov. 4 to pay a $375 hazard-communication citation and reduce a $375 hand-and-power-tool citation to $187.50. The company was issued the citations Oct. 21 during a nonresidential building job at 501 W. Rawson Ave., Oak Creek.
•    Endeavor Hardwoods Inc., Lyndon Station, reached an informal settlement with OSHA on Nov. 4 to reduce a $3,000 fall-protection citation to $1,800 and a $3,000 fall-protection-training citation to $1,800. The company was issued the citations Oct. 28 during a nonresidential building job at 100 E. Industrial Ave., Lyndon Station.
•    Zenith Tech Inc., Waukesha, reached an informal settlement with OSHA on Nov. 5 to reduce two personal-protective-and-life-saving-equipment-working-over-or-near-water citations, totaling $3,250, to $1,625. The company was issued the citations Oct. 20 during a highway and street job in Oconomowoc.
•    J.P. Cullen & Sons Inc., Janesville, reached an informal settlement with OSHA on Nov. 6 to pay a $1,275 stairways citation. The company was issued the citation Oct. 22 during an industrial building and warehouse job at 1000 Bascom Hall, Madison.
•    Dan Romenesko Builders Inc., Appleton, reached an informal settlement with OSHA on Nov. 9 to reduce a $300 fall-protection citation to $210 and a $300 ladders citation to $210. The company was issued the citations Oct. 29 during a single-family housing job in Appleton.
•    U-Haul Co. of Wisconsin Inc., Milwaukee, paid a $1,000 record-keeping-forms citation Nov. 10. The company was issued the citation Sept. 28 during an industrial building and warehouse job at 1570 S. First St., Milwaukee.
•    Sky General LLC, Rothschild, reached an informal settlement with OSHA on Nov. 12 to reduce a $750 rough-terrain-forklift citation to $525 and a $750 scaffolding citation to $525. The company was issued the citations Oct. 30 during a residential remodeling job in Mosinee.
•    Menzel Builders, Tigerton, reached an informal settlement with OSHA on Nov. 12 to reduce a $1,050 scaffolding citation to $400 and a $600 fall-protection citation to $300. The company was issued the citations Oct. 26 during a multifamily housing job in Schofield.
•    Northwoods Custom Builders Inc., Superior, reached an informal settlement with OSHA on Nov. 19 to reduce two fall-protection citations, totaling $750, to $525. The company was issued the citations Nov. 6 during a multifamily housing job at 4316 Third Ave., Superior.
•    A-1 Excavating Inc., Bloomer, and OSHA on Nov. 20 agreed to an administrative law judge ruling to reduce a $7,000 hand-and-power-tools citation to $4,000 and a $70,000 requirements-for-excavation-protective-systems citation to $40,000. The ruling also included dismissal of a $7,000 safety-training-and-education citation, a $7,000 equipment citation and a $7,000 specific-excavation-requirements citation. The company was issued the citations Aug. 11 during a water- and sewer-line job at 660 N. Second St., New Richmond.
•    Badgerland Buildings Inc., Black Creek, reached an informal settlement with OSHA on Nov. 21 to reduce a $1,500 self-propelled-elevating-work-platform citation to $1,050 and a $750 fall-protection citation to $525. The company was issued the citations Nov. 10 during a nonresidential building job in Shawano.
