• Eau Claire Roofing Co. Inc., Eau Claire, must pay four scaffolding citations, totaling $3,000, because the period to contest the violations has ended. The company was issued the citations Nov. 23 during a roofing job at 2653 113th St., Chippewa Falls.
• Cruz’s Roofing & Construction of Madison Inc., Madison, must pay two abatement-verification citations, totaling $800, because the period to contest the violations has ended. The company was issued the citations Nov. 20 after being cited during a roofing job at 3114 Oakridge Road, Madison.
• Lightning Lighting LLC, Glendale, must pay a $1,500 vehicle-mounted-elevating-and-rotating-work-platforms citation and a $1,500 personal-protective-equipment citation because the period to contest the violations has ended. The company was issued the citations Dec. 18 during an electrical job at 6015 S. 60th St., Milwaukee.
• Dale Bennett Construction LLC, Green Bay, must pay a $600 scaffolding citation because the period to contest the violation has ended. The company was issued the citation Nov. 24 during a single-family housing job at 1388 Bingham Drive, De Pere.
• RMP Roofing, Appleton, must pay a $600 eye-and-face-protection citation and a $1,050 fall-protection citation because the period to contest the violations has ended. The company was issued the citations Nov. 24 during a roofing job in Appleton.
• Nationwide Construction Inc., Fort Collins, Colo., reached an informal settlement with OSHA on Dec. 15 to reduce a $1,000 fall-protection citation to $700. The company was issued the citation Nov. 20 during a commercial and institutional building job at 819 W. Shorewood Drive, Eau Claire.
• Primetime Construction LLC, New Glarus, reached an informal settlement with OSHA on Jan. 6 to reduce a $2,000 fall-protection citation to $800 and a $2,000 fall-protection-training citation to $800. The company was issued the citations Dec. 15 during a roofing job at 1100 E. Verona Ave., Verona.
• PTS Contractors Inc., Green Bay, reached an informal settlement with OSHA on Jan. 13 to reduce a $1,225 specific-excavation-requirements citation to $857.50. The company was issued the citation Jan. 6 during a water- and sewer-line job in Oshkosh.
• Lunda Construction Co., Black River Falls, reached an informal settlement with OSHA on Jan. 19 to reduce a $5,000 template-and-pilings citation to $2,500. The company was issued the citation Jan. 8 during a highway, street and bridge job in Oshkosh.
• Lac Courte Oreilles Development Corp., Hayward, reached an informal settlement with OSHA on Jan. 20 to reduce a $500 annual-OSHA-injury-and-illness-survey-of-10-or-more-employers citation to $0. The company was issued the citation Dec. 23.
• Roger Bowers & Sons Construction Co. Inc., paid a $450 specific-excavation-requirements citation Jan. 25. The company was issued the citation Jan. 13 during a water- and sewer-line job in Kaukauna.
• Corporate Contractors Inc., Beloit, reached an informal settlement with OSHA on Jan. 28 to reduce 11 asbestos citations, totaling $29,500, to $16,000. The company and OSHA also agreed to dismiss eight asbestos citations, totaling $20,000. The company was issued the citations Jan. 11 during a nonresidential building job at 690 Third St., Beloit.
• Pahlow Masonry LLC, Shiocton, reached an informal settlement with OSHA on Feb. 1 to reduce a $900 scaffolding citation to $630. The company was issued the citation Jan. 13 during a masonry job at 400 Office Court, Appleton.
• Romi Construction Inc., New Berlin, reached an informal settlement with OSHA on Feb. 2 to reduce a $1,500 fall-protection citation to $1,000 and a $1,500 fall-protection-training citation to $0. The company was issued the citations Jan. 11 during a roofing job at 1718 Moccasin Trail, Waukesha.
• Schweppe Construction, DeForest, reached an informal settlement with OSHA on Feb. 3 to reduce a $525 fall-protection citation to $250. The company was issued the citation Jan. 25 during a carpentry job at 2706 Birchwood Pass, Cross Plains.
• Bechtel Construction Co., San Francisco, reached an informal settlement with OSHA on Feb. 3 to pay a $1,875 electrical-installation-safety-requirements-hazardous-locations citation. The company was issued the citation Jan. 25 during an industrial building job at 11050 S. Chicago Road, Oak Creek.
• Sun Roofing Inc., Appleton, reached an informal settlement with OSHA on Feb. 4 to reduce a $750 fall-protection citation to $525. The company was issued the citation Dec. 22 during a roofing job in Eagle River.
• Appleton Acoustical Systems Inc., Appleton, reached an informal settlement with OSHA on Feb. 4 to reduce a $2,000 fall-protection citation to $1,400. The company was issued the citation Jan. 21 during a specialty trade job at 400 Office Court, Appleton.
• LaLonde Contractors Inc., Milwaukee, paid a $2,500 mechanical-power-transmission-apparatus citation Feb. 8. The company was issued the citation Dec. 21 during a highway and street construction job at 4159 W. Lisbon Ave., Milwaukee.
• Hagen Contractors LLC, Sturtevant, reached an informal settlement with OSHA on Feb. 11 to reduce a $600 power-operated-hand-tools citation to $200 and a $750 fall-protection citation to $350. The company was issued the citations Jan. 25 during a framing job in Kenosha.
• Jose Enriquez Construction, St. Paul, Minn., reached an informal settlement with OSHA on Feb. 11 to reduce a $600 powered-industrial-trucks citation to $300, a $600 eye-and-face-protection citation to $300 and two fall-protection citations, totaling $1,350, to $675. The company was issued the citations Jan. 25 during a roofing job in Hudson.
• Market & Johnson Inc., Eau Claire, reached an informal settlement with OSHA on Feb. 11 to reduce a $3,500 scaffolding citation to $1,400 and to dismiss a $3,500 scaffolding citation. The company was issued the citations Jan. 25 during a nonresidential building job at 314 Jay St., La Crosse.
• 4 Seasons Exteriors Inc., New Berlin, reached an informal settlement with OSHA on Feb. 12 to reduce a $1,500 fall-protection citation to $750 and a $750 fall-protection-training citation to $0. The company and OSHA also agreed to dismiss a $1,500 fall-protection citation and a $750 fall-protection-training citation. The company was issued the citations Jan. 19 during a roofing job at 5018 W. State St., Milwaukee.
• Consolidated Construction Co. Inc., Appleton, reached an informal settlement with OSHA on Feb. 17 to reduce a $1,125 fall-protection citation to $787.50 and to dismiss a $900 stairways-and-ladders citation. The company was issued the citations Jan. 21 during a roofing job at 2204 Crooks Ave., Kaukauna.
• Action Quality Contractors, Beaver Dam, reached a formal settlement with OSHA on Feb. 26 to reduce a $1,050 eye-and-face-protection citation to $420, a $1,500 fall-protection citation to $600, a $1,500 fall-protection-training citation to $600 and a $750 ladders citation to $300. The company was issued the citations Nov. 19 during a roofing job at 121 Milwaukee St., Theresa.
• J&J Construction, Madison, will reach a formal settlement with OSHA on March 12 to reduce a $750 eye-and-face-protection citation to $0, a $750 fall-protection citation to $300 and a $600 fall-protection-training citation to $300. The company was issued the citations Oct. 29 during a roofing job at 3133 Old Gate Road, Madison.
