•    Town City Construction, Appleton, must pay a $600 fall-protection citation because the period to contest the violation has ended. The company was issued the citation Dec. 10 during a roofing job in Eagle River.
•    Kain Roofing LLC, Appleton, must pay a $600 fall-protection citation because the period to contest the violation has ended. The company was issued the citation Jan. 13 during a roofing job at 415 Albert Way, Little Chute.
•    Great Lakes Specialty Contractors Inc., Bonduel, must pay a $500 annual-OSHA-injury-and-illness-survey-of-10-or-more-employers citation because the period to contest the violation has ended. The company was issued the citation Jan. 8.
•    Beaudry Services Inc., Waukesha, must pay a $500 annual-OSHA-injury-and-illness-survey-of-10-or-more-employers citation because the period to contest the violation has ended. The company was issued the citation Jan. 7.
•    Kuhn’s Home Improvements LLC, West Allis, must pay a $750 fall-protection citation, a $600 fall-protection-training citation and a $600 ladders citation because the period to contest the violations has ended. The company was issued the citations Jan. 11 during a roofing job at 142 Highway 99, Mukwonago.
•    Diaz Contractor LLC, Milwaukee, must pay a $750 fall-protection citation and a $750 fall-protection-training citation because the period to contest the violations has ended. The company was issued the citations Jan. 6 during a roofing job at 1308 S. Layton Blvd., Milwaukee.
•    Olympic Builders General Contractors Inc., Holmen, must pay a $1,500 aerial-lifts citation and a $1,500 fall-protection citation because the period to contest the violations has ended. The company was issued the citations Jan. 19 during a multifamily housing job at 13712 Eighth St., Osseo.
•    Glen Johnson Construction, Hudson, must pay a $600 powered-industrial-trucks citation, a $450 head-protection citation and a $450 electrical-wiring-methods citation because the period to contest the violations has ended. The company was issued the citations Jan. 25 during a single-family housing job in Hudson.
•    Tevi Alfano, Middleton, must pay a $600 fall-protection citation and a $600 fall-protection-training citation because the period to contest the violations has ended. The company was issued the citations Jan. 8 during a roofing job at 500 Lancaster Road, Platteville.
•    Burdick Siding & Roofing, Portage, reached a formal settlement with OSHA on Dec. 2 to reduce a $5,600 eye-and-face-protection citation to $560, a $14,000 fall-protection citation to $1,400, a $10,000 fall-protection-training citation to $1,000 and two ladders citations, totaling $5,600, to $560. The company was issued the citations June 11 during a roofing job at 301 Fourth Ave., Baraboo.
•    Gary Overcash Siding LLC, DeForest, reached an informal settlement with OSHA on Feb. 4 to reduce a $1,500 scaffolding citation to $725 and a $1,500 scaffolding-training citation to $725. The company was issued the citations Jan. 26 during a siding job at 2706 Birchwood Pass, Cross Plains.
•    Tri City Glass & Door Inc., Appleton, reached an informal settlement with OSHA on Feb. 9 to reduce a $2,500 scaffolding citation to $1,750. The company was issued the citation Jan. 26 during a glass and glazing job at 1 Pearl Ave., Oshkosh.
•    Masonry Restoration Inc., Milwaukee, reached a formal settlement with OSHA on Feb. 11 to reduce two fall-protection citations, totaling $4,500, to $2,250. The company was issued the citations Nov. 19 during a masonry job at 3 N. Jackson St., Elkhorn.
•    Toonen Companies Inc., Green Bay, reached an informal settlement with OSHA on Feb. 12 to reduce a $1,500 rough-terrain-forklift citation to $1,050 and a $750 fall-protection citation to $525. The company was issued the citations Feb. 1 during an operative building job at 1014 Happy Lane, Sheboygan Falls.
•    R&D Exteriors, Madison, reached an informal settlement with OSHA on Feb. 17 to reduce a $600 fall-protection citation to $420 and a $600 ladders citation to $420. The company was issued the citations Jan. 27 during a roofing job at 4310 Wakefield St., Madison.
•    Bjoin Limestone Inc., Janesville, reached an informal settlement with OSHA on Feb. 17 to reduce three asbestos citations, totaling $3,750, to $1,500. The company was issued the citations Feb. 2 during a wrecking and demolition job at 7308 W. State Road 11, Janesville.
•    Miron Construction Co. Inc., Neenah, reached an informal settlement with OSHA on Feb. 22 to reduce a $1,700 temporary-heating-devices citation to $1,190. The company was issued the citation Feb. 4 during a nonresidential building job in Keshena.
•    Fitchburg Plumbing Inc., Madison, reached an informal settlement with OSHA on Feb. 24 to reduce a $2,000 fire-protection citation to $0 and to pay a $500 stairways citation. The company was issued the citations Feb. 3 during a plumbing, heating and air conditioning job at 402 Genesee St., Delafield.
•    Fox Cities Construction Corp., Oshkosh, reached an informal settlement with OSHA on March 1 to reduce a $375 powered-industrial-trucks citation to $262.50. The company was issued the citation Feb. 4 during a nonresidential building job at 809 S. Commercial St., Neenah.
