OSHA reports

•    Quality Express Roofing, Madison, must pay a $1,500 fall-protection citation, a $1,500 fall-protection-training citation and a $300 abatement-verification citation because the period to contest the violations has ended. The company was issued the citations Dec. 15 and Feb. 19 during and after a roofing job at 602 N. University Ave., Beaver Dam.
•    The OCI Group LLC, Stoughton, must pay a $3,000 fall-protection citation because the period to contest the violation has ended. The company was issued the citation Feb. 18 during a single-family housing job at 4609 Bautista Drive, McFarland.
•    Bay Custom Roofing and Siding Inc., Green Bay, must pay an $800 ladders citation, a $1,600 eye-and-face-protection citation and a $10,000 fall-protection citation following a Feb. 24 review commission ruling. The company was issued the citations July 9 during a roofing job at 1276 Ninth St., Green Bay.
•    Quality Roofing Inc., Marshfield, reached an informal settlement with OSHA on March 5 to reduce a $1,000 flammable-and-combustible-liquids citation to $0 and a $4,000 fall-protection citation to $3,200. The company was issued the citations Feb. 12 during a roofing job at 5370 Highway 10 E., Stevens Point.
•    Professional Roofing & Repair LLC, Monroe, reached an informal settlement with OSHA on March 23 to reduce a $2,800 fall-protection citation to $1,600. The company was issued the citation March 16 during a roofing job at 27569 State Road 81, Cuba City.
•    Ryno’s Masonry, Neillsville, reached an informal settlement with OSHA on March 26 to reduce a $1,500 respiratory-protection citation to $900, a $750 hazard-communication citation to $450, a $1,500 scaffolding-training citation to $900, a $750 ladders citation to $450 and two scaffolding citations, totaling $1,500, to $900. The company was issued the citations March 17 during a masonry job at 9706 County Road H, Marshfield.
•    T.V. John & Son Inc., Butler, reached an informal settlement with OSHA on March 30 to reduce two scaffolding citations, totaling $1,400, to $400 and to dismiss two scaffolding citations, totaling $1,400. The company was issued the citations March 15 during a masonry job at 8129 S. Howell Ave., Oak Creek.
•    B.C.I. of Wauwatosa Inc., Menomonee Falls, reached an informal settlement with OSHA on April 6 to reduce a $900 fall-protection citation to $400 and a $900 ladders citation to $360. The company was issued the citations March 15 during a roofing job at 8129 S. Howell Ave., Oak Creek.
•    North American Framing, Blain, Minn., reached an informal settlement with OSHA on April 7 to reduce a $1,500 fall-protection citation to $900. The company was issued the citation March 23 during a single-family housing job at 37 Bristol Court, Hudson.
•    Dave’s Excavating & Grading Inc., Port Washington, reached an informal settlement with OSHA on April 12 to reduce two ladders citations, totaling $900, to $630. The company was issued the citations Feb. 8 during an excavation job at 2763 S. Shore Drive, Milwaukee.
•    E&W Sewer & Water Construction Inc., Big Bend, reached an informal settlement with OSHA on April 13 to reduce a $1,600 head-protection citation to $800; a $10,000 requirements-for-excavation-protective-systems citation to $5,000; two ladders citations, totaling $1,600, to $0; and three specific-excavation-requirements citations, totaling $6,000, to $2,000. The company was issued the citations March 12 during an excavation job at 8418 94th Court, Pleasant Prairie.
•    Springlake Contracting Inc., Elk Mound, reached an informal settlement with OSHA on April 13 to reduce a $375 liquefied-petroleum-gas citation to $0. The company was issued the citation March 22 during an industrial building and warehouse job in Withee.
•    Schaper Homes Inc., Oconomowoc, reached an informal settlement with OSHA on April 13 to reduce four scaffolding citations, totaling $2,400, to $600. The company was issued the citations March 23 during a single-family housing job at 1418 Hidden River Drive, Mequon.
•    J&J Electric, Menomonie, reached an informal settlement with OSHA on April 15 to reduce a $600 electrical-wiring-methods citation to $420. The company was issued the citation Feb. 9 during an electrical job at 10212 N. Greenwood Lane, Hayward.
•    Marquette Grain Systems Inc., Byron, Minn., reached an informal settlement with OSHA on April 15 to reduce a $750 head-protection citation to $525 and a $1,500 fall-protection citation to $1,050. The company was issued the citations March 23 during a building equipment job at 1300 S. Main St., Alma.
•    Al Hazelton Co., Somerset, paid a $450 head-protection citation April 19. The company was issued the citation Feb. 9 during a finishing job at 121 Third Ave. W., Menomonie.
•    Intercon Construction Inc., Madison, will reach a formal settlement with OSHA on May 6 to dismiss a $2,450 specific-excavation-requirements citation and a $2,450 requirements-for-excavation-protective-systems citation. The company was issued the citations Dec. 9 during an excavation job in Oconomowoc.
•    Barry Loy, Marathon, will reach a formal settlement with OSHA on May 13 to dismiss a $1,500 fall-protection citation, a $750 ladders citation and a $750 eye-and-face-protection citation. The company was issued the citations Oct. 15 during a roofing job at 601 W. Thomas St., Wausau.
