• McHugh Excavating and Plumbing Inc., Onalaska, reached a formal settlement with OSHA on Sept. 23 to reduce a $2,800 safety-training-and-education citation to $1,000; two specific-excavation-requirements citations, totaling $5,600, to $2,000; and three requirements-for-excavation-protective-systems citations, totaling $14,000, to $5,600. The company and OSHA also agreed to dismiss a $2,800 requirements-for-excavation-protective-systems citation. The company was issued the citations May 4, 2009, during an excavation job at 1310 Elmer Road, New Glarus.

• McHugh Excavating and Plumbing Inc., Onalaska, reached a formal settlement with OSHA on Oct. 21 to reduce two specific-excavation-requirements citations, totaling $41,000, to $7,500 and two requirements-for-excavation-protective-systems citations, totaling $52,500, to $7,500. The company was issued the citations Nov. 3, 2008, during a water- and sewer-line job in Madison.

• B.C.I. of Wauwatosa Inc., Menomonee Falls, reached an informal settlement with OSHA on April 6 to reduce a $1,200 eye-and-face-protection citation to $450 and a $3,000 fall-protection citation to $1,350. The company was issued the citations March 30 during a roofing job at 15722 W. Ridge Drive, New Berlin.

• B.C.I. of Wauwatosa Inc., Menomonee Falls, reached an informal settlement with OSHA on April 6 to reduce a $1,125 fall-protection citation to $500. The company was issued the citation March 30 during a roofing job at 2977 S. 10th St., Milwaukee.

• Mid-States Concrete Industries LLC, South Beloit, Ill., reached an informal settlement with OSHA on April 7 to reduce a $1,125 fall-protection citation to $500. The company was issued the citation March 30 during a concrete job at 614 E. Johnson St., Madison.

• RR Walton & Co. Ltd., Whitewater, reached an informal settlement with OSHA on April 9 to reduce a $1,500 fall-protection citation to $1,050 and a $1,500 fall-protection-training citation to $1,050. The company was issued the citations March 30 during a multifamily housing job at 1005 Main St., Whitewater.

• Call’s Metal Building Erectors Inc., Somers, reached an informal settlement with OSHA on April 14 to reduce two fall-protection citations, totaling $1,250, to $875. The company was issued the citations April 1 during a structural steel erection job at 5306 Douglas Ave., Racine.

• P2 Development Co. LLC, Saukville, reached an informal settlement with OSHA on April 15 to reduce a $4,000 fall-protection citation to $2,800 and two ladders citations, totaling $1,425, to $400. The company and OSHA also agreed to dismiss two ladders citations, totaling $1,425. The company was issued the citations April 1 during a multifamily housing job at 501 N. 20th St., Milwaukee.

• R.P.B. Construction Inc., Hartland, reached an informal settlement with OSHA on April 20 to reduce a $750 fall-protection citation to $500 and a $750 fall-protection-training citation to $400. The company was issued the citations April 1 during a multifamily housing job at 4812 N. 104th St., Milwaukee.

• SPS Roofing LLC, Redgranite, reached an informal settlement with OSHA on April 29 to reduce a $1,500 fall-protection citation to $1,050. The company was issued the citation March 30 during a roofing job at 346 N. Casaloma Drive, Appleton.
