• Havenwood Homes Inc., Wind Lake, must pay a $600 fall-protection citation because the period to contest the violation has ended. The company was issued the citation April 13 during a nonresidential building job at S76 W17587 Janesville Road, Muskego.

• Martinez Roofing, Zion, Ill., must pay a $1,500 fall-protection citation and a $600 ladders citation because the period to contest the violations has ended. The company was issued the citations April 13 during a roofing job at 11835 N. Sandhill Circle, Mequon.

• Hip Inc., Waukesha, reached an informal settlement with OSHA on May 4 to reduce two fall-protection citations, totaling $3,000, to $600. The company and OSHA also agreed to dismiss two fall-protection citations, totaling $3,000. The company was issued the citations April 21 during a framing job at 159 W. Jackson St., Milwaukee.

• Northtrack Construction, Fox Point, reached an informal settlement with OSHA on May 8 to reduce a $750 fire-protection citation to $525 and a $525 stairways citation to $367.50. The company was issued the citations May 5 during a nonresidential building job at 4195 W. Bradley Road, Brown Deer.

• Graff Masonry Inc., Jackson, reached an informal settlement with OSHA on May 10 to reduce a $1,050 fall-protection citation to $735 and a $1,050 fall-protection-training citation to $735. The company was issued the citations May 7 during a masonry job at 815 Washington St., Grafton.

• Proline Builders, Caledonia, reached an informal settlement with OSHA on May 11 to reduce a $600 fall-protection citation to $420 and a $1,050 fall-protection-training citation to $735. The company was issued the citations May 7 during a framing job at 9760 S. Bluegrass Place, Oak Creek.

• Kamps Building & Remodeling LLC, Beaver Dam, reached an informal settlement with OSHA on May 13 to reduce a $600 fall-protection citation to $420 and a $600 fall-protection-training citation to $420. The company was issued the citations April 23 during a roofing job at 221 Gould St., Beaver Dam.

• Blackhawk Roofing, Cedar Falls, Iowa, reached an informal settlement with OSHA on May 14 to reduce a $1,500 fall-protection citation to $1,050. The company was issued the citation May 7 during a roofing job at 6401 Copps Drive, Monona.

• J.H. Hassinger Inc., Menomonee Falls, reached an informal settlement with OSHA on May 18 to reduce two scaffolding citations, totaling $1,750, to $400. The company and OSHA also agreed to dismiss two scaffolding citations, totaling $1,750. The company was issued the citations May 5 during a masonry job at 4195 W. Bradley Road, Brown Deer.

• Dick’s Roof Repair Service Inc., Kenosha, reached an informal settlement with OSHA on May 19 to reduce an $800 head-protection citation to $400, an $800 eye-and-face-protection citation to $400, a $4,000 fall-protection citation to $2,000 and a $1,600 fall-protection-training citation to $800. The company was issued the citations May 5 during a roofing job at 7928 39th Ave., Kenosha.

• Applewood Drywall Service Inc., Waukesha, reached an informal settlement with OSHA on May 20 to reduce a $1,250 fire-protection citation to $0 and a $1,750 stairways citation to $875. The company was issued the citations May 6 during a painting and wall-covering job at 4195 W. Bradley Road, Brown Deer.

• Duro-Last Roofing Inc., Saginaw, Mich., reached an informal settlement with OSHA on May 24 to reduce a $1,700 fall-protection citation to $1,190. The company was issued the citation May 11 during a roofing job at 1401 N. Main St., Racine.

• The Kopfmann Co. Inc., Milwaukee, reached an informal settlement with OSHA on May 25 to reduce a $750 fire-protection citation to $0 and a $1,050 stairways citation to $700. The company was issued the citations May 6 during a building equipment job at 4195 W. Bradley Road, Brown Deer.

• Belonger Corp. Inc., West Bend, reached an informal settlement with OSHA on May 26 to reduce a $1,225 stairways citation to $300. The company was issued the citation May 6 during a plumbing, heating and air conditioning job at 4195 W. Bradley Road, Brown Deer.

• Rex A. Carney, Appleton, reached an informal settlement with OSHA on May 27 to reduce a $750 fall-protection citation to $425. The company was issued the citation May 5 during a roofing job in Appleton.

• Richardson Electric Inc., Durand, reached an informal settlement with OSHA on May 28 to reduce a $600 electrical-wiring-methods citation to $420. The company was issued the citation May 11 during an electrical job at 508 E. Monroe Ave., Barron.

• Robert W. Nelson Inc., Milwaukee, reached an informal settlement with OSHA on May 28 to reduce a $375 fire-protection citation to $187.50 and a $525 stairways citation to $262.50. The company was issued the citations May 6 during a framing job at 4195 W. Bradley Road, Brown Deer.

• Horner Plumbing Co. Inc., Pewaukee, reached an informal settlement with OSHA on June 1 to reduce a $1,575 stairways citation to $250. The company was issued the citation May 11 during a plumbing, heating and air conditioning job at 4195 W. Bradley Road, Brown Deer.

• Andy’s Custom Concrete Inc., Elk Mound, paid a $600 liquefied-petroleum-gas citation June 2. The company was issued the citation May 10 during a masonry job at S820 Westland Drive, Spring Valley.

• Albin Construction Inc., Wind Lake, reached an informal settlement with OSHA on June 2 to reduce two aerial-lifts citations, totaling $3,000, to $1,000. The company and OSHA also agreed to dismiss two aerial-lifts citations, totaling $3,000. The company was issued the citations May 11 during a masonry job in Racine.
