• Champion Environmental Services Inc., Gilberts, Ill., must pay a $1,125 fall-protection citation because the period to contest the violation has ended. The company was issued the citation May 12 during a wrecking and demolition job in Milwaukee.

• Siding Brothers Co., Racine, must pay a $2,000 scaffolding citation and a $2,000 scaffolding-training citation because the period to contest the violations has ended. The company was issued the citations May 17 during a siding job at N37 W23880 Broken Hill Circle, Pewaukee.

• Quantum Energy Products Inc., Appleton, reached an informal settlement with OSHA on June 24 to reduce a $750 self-propelled-elevating-work-platform citation to $525, a $1,050 eye-and-face-protection citation to $735 and a $600 electrical-wiring-design-and-protection citation to $420. The company was issued the citations June 14 during a drywall and insulation job in Ledgeview.

• Wayne Nelson Roofing, Stoughton, reached an informal settlement with OSHA on June 25 to reduce a $600 fall-protection citation to $420 and a $600 fall-protection-training citation to $420. The company was issued the citations June 4 during a roofing job at 950 Taylor Lane, Stoughton.

• Hanco Utilities, Lake Nebagamon, Ill., reached an informal settlement with OSHA on June 29 to reduce a $750 ladders citation to $450 and two specific-excavation-requirements citations, totaling $3,000, to $900. The company was issued the citations June 11 during an excavation job in Ashland.

• Swanson Spray Systems Inc., Beloit, reached an informal settlement with OSHA on June 29 to reduce a $750 hand-and-power-tools citation to $525. The company was issued the citation June 9 during a drywall and insulation job at 845 High Ave., Oshkosh.

• Don Hanson Construction, Owen, reached an informal settlement with OSHA on July 1 to reduce a $600 electrical-safety-related-work-practices citation to $300 and a $600 ladders citation to $300. The company was issued the citations June 11 during a specialty trade job at 507 E. Fifth St., Owen.

• Lichtfeld Plumbing Inc., Madison, reached an informal settlement with OSHA on July 6 to reduce a $1,000 requirements-for-excavation-protective-systems citation to $0 and to pay a $1,000 specific-excavation-requirements citation. The company and OSHA also agreed to dismiss a $1,000 requirements-for-excavation-protective-systems citation and a $1,000 specific-excavation-requirements citation. The company was issued the citations June 15 during a plumbing, heating and air conditioning job at 6401 Copps Ave., Monona.

• Peckham Landscaping & Excavation LLC, Windsor, reached an informal settlement with OSHA on July 6 to reduce a $1,000 requirements-for-excavation-protective-systems citation to $0 and to pay a $1,000 specific-excavation-requirements citation. The company and OSHA also agreed to dismiss a $1,000 requirements-for-excavation-protective-systems citation and a $1,000 specific-excavation-requirements citation. The company was issued the citations June 15 during a water- and sewer-line job at 6401 Copps Ave., Monona.

• Tincher Realty Inc., Whitewater, reached an informal settlement with OSHA on July 7 to reduce a $1,050 eye-and-face-protection citation to $630, a $1,500 fall-protection citation to $900 and a $1,500 fall-protection-training citation to $900. The company was issued the citations June 14 during a roofing job at 502 Ventura Lane, Whitewater.

• Cedar Creek Carpentry Inc., West Bend, reached an informal settlement with OSHA on July 7 to reduce a $1,500 rough-terrain-forklift-trucks citation to $400, a $750 powered-industrial-trucks citation to $400, a $750 scaffolding citation to $400, a $750 fall-protection citation to $400, a $750 fall-protection-training citation to $0 and a $600 ladders citation to $400. The company was issued the citations June 17 during a single-family housing job at 2540 N. Calhoun Road, Brookfield.
