• G&R Exteriors LLC, New Berlin, must pay a $1,500 fall-protection citation because the period to contest the violation has ended. The company was issued the citation Aug. 11 during a roofing job at 100 Corrina Blvd., Waukesha.

• Arc Brother Siding Inc., Kenosha, must pay a $1,050 eye-and-face-protection citation, a $1,050 fall-protection citation and a $1,050 ladders citation because the period to contest the violations has ended. The company was issued the citations Aug. 23 during a roofing job at 4601 52nd Ave., Kenosha.

• Woodpecker Hill Construction, Elkhorn, must pay a $600 fall-protection citation and a $600 fall-protection-training citation because the period to contest the violations has ended. The company was issued the citations Aug. 13 during a commercial and institutional building job at 689 Milwaukee St., Burlington.

• Traska Roofing & Ventilation Inc., Wausau, must pay a $750 fall-protection citation and a $750 fall-protection-training citation because the period to contest the violations has ended. The company was issued the citations Aug. 20 during a roofing job at 5606 Jamar St., Schofield.

• Brockman Roofing, Freedom, reached a formal settlement with OSHA on July 7 to reduce a $500 fall-protection citation to $300. The company was issued the citation May 11 during a roofing job in Plymouth.

• Badger Roofing of Wisconsin, Wausau, reached a formal settlement with OSHA on July 12 to reduce a $2,000 aerial-lifts citation to $1,400, a $2,000 eye-and-face-protection citation to $1,000 and two fall-protection citations, totaling $7,600, to $2,400. The company and OSHA also agreed to dismiss a $2,000 electrical-wiring-methods citation. The company was issued the citations Dec. 17 during a roofing job at 1525 Townline Road, Wausau.

• R.G. Huston Co. Inc., Cottage Grove, reached an informal settlement with OSHA on Sept. 16 to reduce two respiratory-protection citations, totaling $2,400, to $1,680. The company was issued the citations Sept. 8 during a water- and sewer-line job at 5320 Monona Drive, Madison.

• Ad Roofing LLC, Sussex, reached an informal settlement with OSHA on Sept. 22 to reduce a $1,500 fall-protection citation to $750 and a $450 ladders citation to $225. The company was issued the citations Sept. 10 during a roofing job at 2938 Summit Ave., Milwaukee.

• GTO Contracting, Neenah, reached an informal settlement with OSHA on Sept. 22 to reduce a $600 fall-protection citation to $420. The company was issued the citation Sept. 13 during a roofing job at 1223 Stead Drive, Menasha.

• Azco Inc., Appleton, reached a formal settlement with OSHA on Sept. 22 to pay a $1,875 confined-spaces citation and reduce a $1,875 electrical-installation-safety-requirements citation to $950. The company was issued the citations Jan. 20 during a heavy construction job at 11050 S. Chicago Road, Oak Creek.

• TNT Roofing, Delavan, reached an informal settlement with OSHA on Sept. 22 to reduce a $1,500 fall-protection citation to $900 and a $1,500 fall-protection-training citation to $900. The company was issued the citations Sept. 2 during a roofing job at 975 Main St., Whitewater.

• Madison Crushing & Excavating Co. Inc., Madison, reached an informal settlement with OSHA on Sept. 23 to dismiss a $2,000 safety-training-and-education citation and to pay a $4,000 requirements-for-excavation-protective-systems citation. The company was issued the citations Sept. 8 during an excavation job in Madison.

• R.J.’s West Side Demolition Inc., Friendship, reached an informal settlement with OSHA on Sept. 23 to reduce a $750 fall-protection citation to $525. The company was issued the citation Sept. 20 during a wrecking and demolition job at 95 N. Moorland Road, Brookfield.

• Rezin Construction Ltd., Tomah, reached an informal settlement with OSHA on Sept. 27 to reduce a $1,500 specific-excavation-requirements citation to $1,050 and a $1,500 requirements-for-excavation-protective-systems citation to $1,050. The company was issued the citations Sept. 16 during an excavation job in Tomah.

• The Spancrete Group Inc., Waukesha, reached an informal settlement with OSHA on Sept. 28 to reduce a $2,000 ladders citation to $1,000 and to pay a $2,000 fall-protection citation. The company and OSHA also agreed to dismiss two fall-protection citations, totaling $4,000. The company was issued the citations Sept. 14 during a concrete job at 212 S. Barclay St., Milwaukee.

• Universal Masonry Construction LLC, Kenosha, reached an informal settlement with OSHA on Sept. 28 to reduce a $1,500 occupational-noise-exposure citation to $800; a $1,500 gases-vapors-fumes-dusts-and-mists citation to $800; a $1,500 respiratory-protection citation to $800; a $1,500 aerial-lifts citation to $0; a $1,500 scaffolding-training citation to $0; a $1,500 hand-and-power-tools citation to $1,000; three hazard-communication citations, totaling $3,150, to $550; and six scaffolding citations, totaling $9,000, to $3,000. The company and OSHA also agreed to dismiss a $1,500 aerial-lifts citation; three hazard-communication citations, totaling $3,150; and five scaffolding citations, totaling $7,500. The company was issued the citations Sept. 16 during a masonry job at 5038 19th Ave., Kenosha.

• Moraine Builders Inc., Sheboygan Falls, reached an informal settlement with OSHA on Sept. 29 to reduce a $300 fall-protection citation to $210. The company was issued the citation Sept. 10 during a single-family housing job at W3329 Sunset Road, Sheboygan Falls.

• Lonnie’s Roofing & Siding, South Milwaukee, reached an informal settlement with OSHA on Sept. 29 to reduce a $750 fall-protection citation to $375 and a $600 ladders citation to $200. The company was issued the citations Sept. 15 during a roofing job at 813 Andover Drive, Eagle.

• MJW Enterprises LLC, Eau Claire, reached an informal settlement with OSHA on Sept. 30 to reduce a $450 head-protection citation to $225. The company was issued the citation Sept. 3 during a single-family housing job at 4311 Echo Valley Drive, Eau Claire.

• Morgan Contracting Inc., Albertville, Minn., reached an informal settlement with OSHA on Oct. 4 to reduce two requirements-for-excavation-protective-systems citations, totaling $6,000, to $4,200. The company was issued the citations Sept. 8 during a water- and sewer-line job in Madison.

• Rick Zellmer Construction, Muskego, paid a $450 fall-protection citation Oct. 5. The company was issued the citation Aug. 11 during a roofing job at 3025 N. 124th St., Wauwatosa.

• Waunakee Remodeling Inc., Waunakee, reached an informal settlement with OSHA on Oct. 5 to reduce a $1,000 eye-and-face-protection citation to $590 and a $2,500 fall-protection citation to $1,475. The company was issued the citations Sept. 16 during a roofing job at 614 Chapman St., Madison.

• La Contractors LLC, Milwaukee, reached an informal settlement with OSHA on Oct. 12 to reduce a $750 fall-protection-training citation to $0 and to pay a $750 fall-protection citation. The company and OSHA also agreed to dismiss a $750 fall-protection citation and a $750 fall-protection-training citation. The company was issued the citations Sept. 1 during a roofing job at 4103 N. Downer Ave., Shorewood.

• Zenith Tech Inc., Waukesha, and OSHA on Oct. 17 agreed to a review a commission ruling to reduce a $4,000 concrete-and-masonry-construction citation to $0. The company was issued the citation June 21 during a highway and street job in Superior.

• Interstate Roofing & Waterproofing Inc., Onalaska, reached a formal settlement with OSHA on Oct. 22 to reduce a $5,600 fall-protection citation to $4,000. The company was issued the citation June 2 during a roofing job at 2105 Palace St., La Crosse.

• D’Alie Masonry LLC, Rochester, will reach a formal settlement with OSHA on Oct. 30 to reduce a $1,400 head-protection citation to $196.26, a $2,000 eye-and-face-protection citation to $280.37 and six scaffolding citations, totaling $18,000, to $2,523.36. The company was issued the citations June 5, 2009, during a masonry job at 2210 Lathrop Ave., Racine.
