• RJS Contracting, Milwaukee, must pay a $750 scaffolding citation and a $750 fall-protection citation because the period to contest the violations has ended. The company was issued the citations Sept. 10 during a roofing job at 2756 S. Howell Ave., Milwaukee.

• Superior Surfacing Corp., Greenfield, must pay a $750 respiratory-protection citation, a $750 methylene chloride citation, a $750 hazard-communication citation, a $450 personal-protective-equipment citation and a $600 flammable-and-combustible-liquids citation because the period to contest the violations has ended. The company was issued the citations Sept. 3 during a specialty trade job in Milwaukee.

• David and Goliath Builders Inc., Pewaukee, reached an informal settlement with OSHA on Sept. 7 to reduce a $1,500 elevated-platform citation to $900, a $1,500 powered-industrial-trucks citation to $0, a $1,500 scaffolding citation to $900 and a $600 stairways citation to $0. The company also will reach a formal settlement with OSHA on Dec. 10 to dismiss a $1,500 truck citation and to reduce a $1,500 elevated-platform citation to $600. The company was issued the citations Aug. 13 during a framing job in West Allis.

• Applied Flooring Solutions LLC, Green Bay, reached an informal settlement with OSHA on Nov. 3 to reduce a $375 fire-protection citation to $262. The company was issued the citation Oct. 28 during a flooring job at 3600 S. Moorland Road, New Berlin.

• Oak Roofing Inc., Wood Dale, Ill., reached an informal settlement with OSHA on Nov. 3 to reduce a $16,500 fall-protection citation to $8,250 and a $16,500 fall-protection-training citation to $8,250. The company was issued the citations Oct. 26 during a roofing job at 174 State Road 50, Delavan.

• Capelle Bros. & Diedrich Inc., Fond du Lac, reached an informal settlement with OSHA on Nov. 3 to reduce a $525 criteria-for-personal-protective-equipment citation to $367.50. The company was issued the citation Oct. 26 during a nonresidential building job at 91 N. Pioneer Road, Fond du Lac.

• Freimuth Masonry Inc., Appleton, reached an informal settlement with OSHA on Nov. 4 to reduce a $1,000 occupational-noise-exposure citation to $700, a $1,000 eye-and-face-protection citation to $700 and a $1,000 fall-protection citation to $700. The company was issued the citations Nov. 1 during a masonry job in Appleton.

• Vanguard Contracting LLC, Oshkosh, reached an informal settlement with OSHA on Nov. 8 to reduce a $750 eye-and-face-protection citation to $525, a $1,500 fall-protection citation to $1,050 and a $750 powered-industrial-trucks citation to $525. The company was issued the citations Oct. 27 during a residential building job in Fond du Lac.

• Fernando Mancera, Richland Hills, Texas, reached an informal settlement with OSHA on Nov. 10 to reduce a $750 eye-and-face-protection citation to $450, a $1,500 fall-protection citation to $900 and a $600 ladders citation to $360. The company was issued the citations Oct. 28 during a roofing job at 512 Fifth St., Menasha. The company also reached an informal settlement with OSHA on Nov. 10 to reduce a $3,000 fall-protection citation to $1,500 and a $2,400 ladders citation to $1,200. The company was issued the citations Oct. 28 during a roofing job at 1327 W. College Ave., Appleton.

• Mike Lyster Painting & Wallcovering Inc., Oshkosh, reached an informal settlement with OSHA on Nov. 10 to reduce a $1,500 aerial-lifts citation to $800. The company was issued the citation Nov. 1 during a painting and wall covering job at 620 Elmwood Ave., Oshkosh.

• Ver Halen Inc., Green Bay, reached an informal settlement with OSHA on Nov. 15 to reduce a $1,375 fire-protection citation to $962.50. The company was issued the citation Oct. 28 during a building equipment job at 3600 S. Moorland Road, New Berlin.

• Robert W. Nelson Inc., Milwaukee, reached an informal settlement with OSHA on Nov. 16 to reduce a $625 fire-protection citation to $400 and a $625 fall-protection citation to $200. The company was issued the citations Oct. 28 during a framing job at 3600 S. Moorland Road, New Berlin.

• RB Roofing & Siding, Oshkosh, reached an informal settlement with OSHA on Nov. 16 to reduce a $750 eye-and-face-protection citation to $450 and a $750 fall-protection citation to $450. The company was issued the citations Oct. 27 during a roofing job in Appleton.

• Ebert Roofing LLC, Fall River, reached an informal settlement with OSHA on Nov. 16 to reduce a $4,620 fall-protection citation to $3,234. The company was issued the citation Nov. 2 during a roofing job in Columbus.

• A-1 Excavating Inc., Bloomer, reached an informal settlement with OSHA on Nov. 16 to reduce a $4,200 saw-kickbacks citation to $2,100 and a $1,500 equipment citation to $1,050. The company and OSHA also agreed to dismiss a $4,200 safety-training-and-education citation. The company was issued the citations Nov. 2 during a water- and sewer-line job in Osseo.

• Rodriguez Quality Roofing, Madison, reached an informal settlement with OSHA on Nov. 17 to reduce a $2,000 fall-protection citation to $1,350 and a $2,000 ladders citation to $1,350. The company was issued the citations Oct. 26 during a roofing job at 1 W. Main St., Evansville.

• Cuellar Construction, West Bend, reached an informal settlement with OSHA on Nov. 18 to reduce a $3,000 eye-and-face-protection citation to $1,500, a $2,100 ladders citation to $1,050 and a $4,200 fall-protection citation to $3,000. The company was issued the citations Oct. 27 during a roofing job in Elkhart Lake.

• Giraffe Electric II Inc., Waukesha, paid a $375 fire-protection citation Nov. 19. The company was issued the citation Oct. 28 during an electrical job at 3600 S. Moorland Road, New Berlin.

• Luecke Siding and Roofing, Howards Grove, reached an informal settlement with OSHA on Nov. 22 to reduce a $750 eye-and-face-protection citation to $350 and a $1,500 fall-protection citation to $850. The company was issued the citations Oct. 27 during a roofing job at 1120B Aspen Court, Kohler.

• Sendik’s Managements Co. Inc., Whitefish Bay, reached an informal settlement with OSHA on Nov. 22 to reduce a $750 fire-protection citation to $525. The company was issued the citation Oct. 28 during an operative building job at 3600 S. Moorland Road, New Berlin.

• Spancrete Inc., Waukesha, reached an informal settlement with OSHA on Nov. 24 to reduce a $1,500 fall-protection citation to $750. The company was issued the citation Nov. 1 during a specialty trade job in Appleton.

• Werlein Siding and Roofing, Durand, reached an informal settlement with OSHA on Nov. 24 to reduce a $1,800 eye-and-face-protection citation to $0 and a $3,000 fall-protection citation to $0. The company was issued the citations Nov. 1 during a roofing job at 2801 Richard Drive, Eau Claire.

• Gethsemane Roofing, McFarland, reached an informal settlement with OSHA to reduce a $4,200 fall-protection citation $2,000 and a $3,600 eye-and-face-protection citation to $0. The company and OSHA also agreed to dismiss a $3,600 eye-and-face-protection citation and a $4,200 fall-protection citation. The company was issued the citations Nov. 3 during a residential remodeling job at 220 Sunrise Court, Deerfield.

• Windy Ridge Construction, Barneveld, reached an informal settlement with OSHA on Dec. 1 to reduce a $1,500 fall-protection citation to $1,050. The company was issued the citation Oct. 15 during a single-family housing job in Sun Prairie.
