• Martha Alvarez, Houston, must pay a $3,600 fall-protection citation because the period to contest the violation has ended. The company was issued the citation Dec. 10 during a residential remodeling job at 21 Foxboro Circle, Madison. The company also must pay a $1,800 head-protection citation, a $1,800 disposal-of-waste-materials citation and a $4,200 fall-protection citation because the period to contest the violations has ended. The company was issued the citations Dec. 29 during a roofing job at 5210 Brookside Drive, Madison.
• Jesus Marufo, St. Charles, Mo., must pay a $4,200 fall-protection citation and a $4,200 fall-protection-training citation because the period to contest the violations has ended. The company was issued the citations Dec. 8 during a roofing job at 1463 Business Park Road, Mineral Point.
• Crafts Inc., Manitowoc, reached a formal settlement with OSHA on Feb. 7 to dismiss a $1,500 fall-protection citation and to reduce two fall-protection citations, totaling $3,000, to $2,250. The company was issued the citations April 9 during a roofing job at 340 Manitowoc St., Reedsville.
• Dr. Restoration, Elk Grove, Ill., reached an informal settlement with OSHA on Feb. 7 to reduce a $3,240 head-protection citation to $2,268, a $3,240 eye-and-face-protection citation to $2,268 and a $3,780 fall-protection citation to $2,646. The company was issued the citations Jan. 26 during a nonresidential building job at 4202 East Towne Mall, Madison.
• Chilstrom Erecting Corp., West Milwaukee, reached an informal settlement with OSHA on Feb. 9 to reduce a $2,677 aerial-lifts citation to $1,300. The company was issued the citation Jan. 27 during a structural-steel erection job at 515 W. Moreland Blvd., Waukesha.
• USA Hoist Corp., Chicago, reached an informal settlement with OSHA on Feb. 17 to pay a $4,200 scaffolding citation and to reduce three working-over-or-near-water citations, totaling $7,650, to $3,000. The company and OSHA also agreed to dismiss two working-over-or-near-water citations, totaling $5,100. The company was issued the citations Jan. 28 during a specialty trade job at 735 N. Water St., Milwaukee.
