• Pauly Alverez, Lincoln, Neb., must pay a $3,600 head-protection citation, a $3,600 eye-and-face-protection citation, a $3,000 disposal-of-waste-materials citation, a $4,200 fall-protection citation and a $4,200 fall-protection-training citation because the period to contest the violations has ended. The company was issued the citations Feb. 15 during a roofing job at 5442 Coachman Place, Madison.

• PR Roofing & Construction, Madison, must pay a $600 abatement-verification citation because the period to contest the violation has ended. The company was issued the citation Feb. 17 following a nonresidential building job in Madison.

• B&D Erectors Inc., Weatherford, Texas, must pay a $4,200 self-propelled-elevating-work-platform citation because the period to contest the violation has ended. The company was issued the citation Feb. 15 during a siding job at 4010 Marinette Ave., Marinette.

• EZ Roofing, Sheboygan, must pay a $3,000 asbestos citation because the period to contest the violation has ended. The company was issued the citation Feb. 15 during a roofing job at 1903 Union Ave., Sheboygan.

• Julio Y Guamuche, Appleton, must pay a $4,200 fall-protection citation because the period to contest the violation has ended. The company was issued the citation Feb. 17 during a roofing job at 4616 Town Lake Circle, Grand Chute.

• Alpine Insulation Co. Inc., Sheboygan, reached an informal settlement with OSHA on March 29 to reduce a $4,200 fall-protection citation to $2,100 and two scaffolding citations, totaling $8,400, to $2,100. The company and OSHA also agreed to dismiss two scaffolding citations, totaling $8,400. The company was issued the citations March 11 during a framing job at 1508 Mohican Trail, Waukesha.

• T&J Concrete Foundations Inc., Cadott, reached an informal settlement with OSHA on March 29 to reduce a $2,618 scaffolding citation to $1,048. The company was issued the citation March 11 during a concrete job at 522 Water St., Eau Claire.

• Solais Siding, Sturtevant, reached an informal settlement with OSHA on April 5 to reduce a $3,960 stairways-and-ladders-training citation to $500. The company was issued the citation March 16 during a siding job in Pleasant Prairie.

• J.H.W. Builders LLC, Milwaukee, reached an informal settlement with OSHA on April 6 to reduce a $2,700 scaffolding citation to $1,200. The company was issued the citation March 16 during a framing job at 4880 Providence Drive, New Berlin.

• Davis & Associates Builders LLC, Lancaster, reached an informal settlement with OSHA on April 8 to reduce a $3,600 eye-and-face-protection citation to $1,800, a $3,600 fall-protection citation to $1,800, a $3,600 fall-protection-training citation to $1,800 and a $2,400 ladders citation to $1,200. The company was issued the citations March 17 during a roofing job at 1130 N. Johns St., Dodgeville.

• Last Drop Roofing, Siding & Seamless Gutters LLC, Two Rivers, reached a formal settlement with OSHA on April 11 to reduce a $5,280 scaffolding citation to $2,250 and a $3,960 fall-protection citation to $2,250. The company was issued the citations Oct. 12 during a roofing job at 2140 34th St., Two Rivers.

• Ridge Top Roofing and Siding Inc., Madison, reached a formal settlement with OSHA on April 21 to reduce a $3,780 fall-protection citation to $500. The company was issued the citation Jan. 26 during a roofing job at 5210 and 5218 Brookside Drive, Madison.
