• F&H Drywall LLC, Milwaukee, must pay a $2,400 ladders citation and a $2,400 stairways-and-ladders-training citation because the period to contest the violations has ended. The company was issued the citations March 15 during a drywall and insulation job at 3530 Straubel St., Madison.

• J&H Heating Inc., Port Washington, reached a formal settlement with OSHA on Feb. 11 to dismiss an $875 fall-protection citation. The company was issued the citation Oct. 5 during a plumbing, heating and air conditioning job at 6800 60th St., Kenosha.

• D&B Industrial Floor Coatings Inc., Germantown, reached a formal settlement with OSHA on March 31 to dismiss a $1,575 electrical-wiring-design-and-protection citation and a $900 electrical-wiring-methods citation. The company was issued the citations Oct. 13 during a flooring job at 5030 S. 74th St., Greenfield.

• Dnesco Electric Inc., Greenfield, reached an informal settlement with OSHA on April 14 to reduce a $2,800 fall-protection-training citation to $700. The company was issued the citation April 5 during an electrical job at 1330 52nd St., Kenosha.

• Hans Roofing & Siding Inc., Oconomowoc, reached an informal settlement with OSHA on April 14 to reduce a $1,800 head-protection citation to $300 and to dismiss a $2,400 fall-protection citation. The company was issued the citations April 6 during a roofing job in West Allis.

• Langer Roofing & Sheet Metal Inc., Milwaukee, reached an informal settlement with OSHA on April 21 to reduce a $3,442 fall-protection-training citation to $3,422. The company was issued the citation April 5 during a roofing job at 2801 80th St., Kenosha.

• Bohemia Construction Inc., Milwaukee, reached an informal settlement with OSHA on April 21 to reduce a $2,160 fall-protection citation to $1,300 and a $2,160 ladders citation to $0. The company was issued the citations April 6 during a roofing job at N42 W22656 Brighton Court, Pewaukee.

• Robert P. O’Haver, Oconomowoc, reached an informal settlement with OSHA on April 22 to reduce a $2,400 aerial-lifts citation to $1,680. The company was issued the citation April 8 during a siding job at 1540 N. 116th St., Wauwatosa.

• Lunda Construction Co., Black River Falls, reached an informal settlement with OSHA on April 25 to reduce a $5,100 safety-training-and-education citation to $3,000 and to dismiss a $5,100 scaffolding citation. The company was issued the citations March 31 during a highway, street and bridge job in Big Bend.

• McCarville Construction LLC, Lancaster, reached an informal settlement with OSHA on April 26 to reduce a $2,400 ladders citation to $1,200, a $3,600 fall-protection citation to $1,800 and a $3,600 fall-protection-training citation to $0. The company and OSHA also agreed to dismiss a $3,600 fall-protection citation and a $3,600 fall-protection-training citation. The company was issued the citations April 18 during a roofing job at 670 N. Washington St., Platteville.

• VJS Construction Services Inc., Pewaukee, reached an informal settlement with OSHA on April 26 to pay a $3,442 fall-protection citation. The company was issued the citation April 5 during an industrial building job at 2801 80th St., Kenosha.

• Alpine Construction Services Inc., Chicago, reached an informal settlement with OSHA on April 28 to reduce a $2,400 fire-protection citation to $0, a $3,000 electrical-wiring-design-and-protection citation to $1,500 and a $3,000 electrical-wiring-methods citation to $0. The company was issued the citations April 1 during a flooring job in Muskego.

• ELW Corp., Dubuque, Iowa, reached an informal settlement with OSHA on April 28 to reduce a $4,200 safety-training-and-education citation to $2,940 and a $4,200 requirements-for-excavation-protective-systems citation to $2,940. The company was issued the citations April 18 during a concrete job at 401 S. Main St., Cuba City.

• PTS Contractors Inc., Green Bay, reached an informal settlement with OSHA on April 29 to reduce two requirements-for-excavation-protective-systems citations, totaling $10,780, to $0 and three specific-excavation-requirements citations, totaling $21,560, to $13,000. The company and OSHA also agreed to dismiss a $5,390 specific-excavation-requirements citation. The company was issued the citations April 8 during an excavation job in Plymouth.

• Camosy Inc., Kenosha, reached an informal settlement with OSHA on April 29 to pay a $2,142 fall-protection citation. The company was issued the citation April 5 during a nonresidential building job at 1330 52nd St., Kenosha.

• Brick Stone LLC, Watertown, reached an informal settlement with OSHA on May 4 to reduce two scaffolding citations, totaling $5,100, to $1,600. The company was issued the citations April 19 during a masonry job at 2863 Heritage Drive, Delafield.

• John Schmitz, Milwaukee, reached an informal settlement with OSHA on May 4 to reduce a $3,600 fall-protection citation to $1,800 and $2,400 ladders citation to $0. The company and OSHA also agreed to dismiss a $3,600 fall-protection citation and a $2,400 ladders citation. The company was issued the citations March 30 during a roofing job at 5521 N. Santa Monica Blvd., Whitefish Bay.

The company and OSHA also agreed to dismiss two hand-and-power-tools citations, totaling $10,780. The company was issued the citations April 14 during a framing job in Muskego.

• B&G Roofing Inc., Butler, reached an informal settlement with OSHA on May 10 to reduce a $1,530 scaffolding citation to $1,071. The company was issued the citation March 31 during a roofing job at 2221 E. Oklahoma Ave., Milwaukee.

• C.D. Smith Construction Inc., Fond du Lac, reached an informal settlement with OSHA on May 10 to reduce a $2,550 means-of-egress citation to $1,275, a $4,250 fire-protection citation to $2,125 and a $3,400 electrical-wiring-methods citation to $0. The company and OSHA also agreed to dismiss a $3,400 general-safety-and-health-provisions citation. The company was issued the citations April 19 during an industrial building and warehouse job in Muskego.
