• Alberto Miranda Roofing, Hereford, Texas, must pay a $3,600 head-protection citation, a $3,600 eye-and-face-protection citation, a $4,200 fall-protection citation, a $4,200 fall-protection-training citation and a $4,200 ladders citation because the period to contest the violations has ended. The company was issued the citations June 6 during a roofing job at 202 23rd St., La Crosse.
• Julian Mendoza, Palatine, Ill., must pay a $1,800 head-protection citation, a $3,600 eye-and-face-protection citation, a $1,800 disposal-of-waste-materials citation and a $4,200 fall-protection citation because the period to contest the violations has ended. The company was issued the citations June 6 during a roofing job on Watts Road in Madison.
• Jose Conejo, Fort Worth, Texas, must pay a $3,600 fall-protection citation, a $3,600 fall-protection-training citation and a $2,400 ladders citation because the period to contest the violations has ended. The company was issued the citations June 6 during a roofing job at 8501 Elderberry Road, Madison.
• Tim Brieske, Milton, must pay a $4,200 fall-protection citation and a $4,200 fall-protection-training citation because the period to contest the violations has ended. The company was issued the citations June 6 during a roofing job at 616 Milton Ave., Janesville.
• Rawson Contractors Inc., Sussex, reached a formal settlement with OSHA on July 9, 2010, to reduce a $750 specific-excavation-requirements citation to $100 and to dismiss a $1,000 specific-excavation-requirements citation and a $1,250 requirements-for-excavation-protective-systems citation. The company was issued the citations Nov. 23, 2009, during a water- and sewer-line job at 4400 Industrial Park Drive, Stevens Point.
• Architectural Renovators LLC, Menasha, must pay a $1,050 specific-excavation-requirements citation following a Dec. 13, 2010, review commission ruling. The company was issued the citation April 15, 2010, during an industrial building and warehouse job at 230 Main St., Menasha.
• Masonry Restoration Inc., Milwaukee, reached an informal settlement with OSHA on July 6 to reduce a $5,390 aerial-lifts citation to $3,773 and a $3,850 fall-protection citation to $2,695. The company was issued the citations June 28 during a masonry job at 700 57th St., Kenosha.
• K&L Construction SW LLC, Bloomington, reached an informal settlement with OSHA on July 6 to reduce a $3,600 eye-and-face-protection citation to $2,100 and a $3,600 fall-protection citation to $2,100. The company was issued the citations June 28 during a roofing job at 1000 N. Adams St., Lancaster.
• Exterio’s Expert’s Restoration, Houston, reached an informal settlement with OSHA on July 8 to reduce a $4,200 fall-protection citation to $2,940. The company was issued the citation June 29 during a roofing job on Windsor Commons Avenue in Windsor.
• Advance Construction Inc., Green Bay, reached an informal settlement with OSHA on July 8 to reduce a $3,850 specific-excavation-requirements citation to $2,000, a $3,080 ladders citation to $0 and two requirements-for-excavation-protective-systems citations, totaling $10,780, to $2,700. The company was issued the citations June 28 during a water- and sewer-line job in Green Bay.
• Simmons Roofing LLC, Alma Center, reached an informal settlement with OSHA on July 11 to reduce a $2,400 fall-protection citation to $960. The company was issued the citation June 30 during a roofing job at 2221 Bennora Lee Court, La Crosse.
• Midwest Plumbing LLC, Waukesha, reached an informal settlement with OSHA on July 19 to reduce a $2,400 specific-excavation-requirements citation to $0 and a $2,400 requirements-for-excavation-protective-systems citation to $1,200. The company was issued the citations June 22 during a plumbing, heating and air conditioning job at 15400 W. North Ave., Brookfield.
