• Carlos Victor, Menasha, must pay a $4,200 fall-protection citation and a $4,200 fall-protection-training citation because the period to contest the violations with a Wisconsin OSHA office has ended. The company was issued the citations Jan. 19 during a roofing job at 8846 Greenway Blvd., Middleton.
• GTO Contractors LLC, Neenah, must pay a $3,850 general-safety-and-health-provisions citation, a $46,200 fall-protection citation, a $10,780 fall-protection-training citation and two abatement-verification citations, totaling $1,540, because the period to contest the violations with a Wisconsin OSHA office has ended. The company was issued the citations March 20 and May 22 during and after a roofing job at 8628 Greenway Blvd., Middleton.

• R.J. Kampo Plumbing & Heating Inc., Appleton, reached an informal settlement with OSHA on June 8 to reduce a $3,000 fall-protection citation to $1,500 and a $1,800 stairways-and-ladders citation to $0. The company and OSHA also agreed to dismiss a $1,800 electrical-safety-related-work-practices citation. The company was issued the citations May 25 during a plumbing, heating and air conditioning job at N172 Highway 47/57, Keshena.
• Jen Industries LLC, Fredonia, will reach a formal settlement with OSHA on July 7 to reduce seven lead citations, totaling $21,000, to $7,500. The company and OSHA also agreed to dismiss two lead citations, totaling $6,000. The company was issued the citations Nov. 18 during an industrial building and warehouse job at 5000 W. National Ave., Milwaukee.
• Town & Country Underground Utility Construction Inc., Mayville, will reach a formal settlement with OSHA on July 8 to reduce a $26,950 requirements-for-excavation-protective-systems citation to $9,625. The company was issued the citation Sept. 1 during an excavation job in Mayville.
