• Inspired Home Improvements, West Allis, must pay a $1,200 eye-and-face-protection citation, a $2,800 fall-protection citation and a $2,800 fall-protection-training citation because the period to contest the violations with a Wisconsin OSHA office has ended. The company was issued the citations May 30 during a roofing job at 2761 S. Wentworth Ave., Milwaukee.

• C&L Contractors LLC, Racine, must pay a $1,800 eye-and-face-protection citation, a $2,400 fall-protection citation, a $2,400 fall-protection-training citation and three scaffolding citations, totaling $7,200, because the period to contest the violations with a Wisconsin OSHA office has ended. The company was issued the citations June 8 during a siding job at 7313 Beloit St., West Allis.

• DTK Builders LLC, Monroe, must pay a $2,400 fall-protection citation and a $2,400 fall-protection-training citation because the period to contest the violations with a Wisconsin OSHA office has ended. The company was issued the citations June 11 during a roofing job at 102 E. Mills Drive, Lake Mills.

• A-Plus Contractors of Wisconsin Inc., Greenfield, reached a formal settlement with OSHA on June 18 to pay a $3,000 aerial-lifts citation and a $4,200 fall-protection citation. The company was issued the citations April 13, 2011, during a roofing job at 6801 W. Loomis Road, Greendale.

• Shun Yu Li Inc., Carol Stream, Ill., reached an informal settlement with OSHA on June 26 to reduce a $1,600 fall-protection citation to $1,120 and a $1,600 fall-protection-training citation to $1,120. The company was issued the citations June 13 during a siding job at 1109 Tenny Ave., Waukesha.

• J&J Contractors I LLC, Oak Creek, reached an informal settlement with OSHA on June 26 to reduce a $1,445 fall-protection citation to $1,011.50. The company was issued the citation June 18 during a roofing job at 432 S. Kane St., Burlington.

• Coppens Inc., Green Bay, reached an informal settlement with OSHA on July 2 to reduce a $1,122 head-protection citation to $1,000 and a $4,400 fall-protection citation to $2,000. The company was issued the citations June 7 during a roofing job at 600 W. Webster Ave., Green Bay.

• Higher Ground Roofing Inc., Kewaskum, reached an informal settlement with OSHA on July 2 to reduce a $2,000 fall-protection citation to $1,400. The company was issued the citation June 18 during a roofing job at 1870 27th Ave., Kenosha.

• Koziczkowski Construction, Stevens Point, reached an informal settlement with OSHA on July 3 to reduce a $1,600 fall-protection citation to $900. The company was issued the citation June 25 during a roofing job at 5224 Heffron St., Stevens Point.

• Team Green Geothermal, Kimball, Minn., reached an informal settlement with OSHA on July 5 to reduce a $2,000 requirements-for-excavation-protective-systems citation to $1,000. The company was issued the citation June 7 during a water- and sewer-line job at 1111 Clairemont Ave., Eau Claire.
