  • Creative General Contractors LLC, Milwaukee, must pay a $400 abatement-verification citation because the period to contest the violation with a Wisconsin OSHA office has ended. The company was issued the citation Sept. 30 in connection with a residential remodeling job at 10330 Buckingham Drive, Oak Creek.
  • Solais Siding, Sturtevant, must pay a $400 abatement-verification citation because the period to contest the violation with a Wisconsin OSHA office has ended. The company was issued the citation Oct. 18 in connection with a siding job at 7080 W. Concord Creek Drive, Mequon.
  • Carlos Ketz, Appleton, must pay a $4,000 fall-protection citation and a $4,000 fall-protection-training citation because the period to contest the violation with a Wisconsin OSHA office has ended. The company was issued the citations Sept. 25 during a roofing job at 7615 Mission Drive, Franklin.
  • K&L Roofing LLC, Sparta, must pay a $400 fall-protection-training citation because the period to contest the violation with a Wisconsin OSHA office has ended. The company was issued the citation Oct. 25 in connection with a roofing job at 13477 Inspiration Ave., Sparta.
  • Ed’s Construction Inc., Richfield, Minn., must pay a $2,800 fall-protection citation and a $2,800 fall-protection-training citation because the period to contest the violations with a Wisconsin OSHA office has ended. The company was issued the citations Oct. 31 during a roofing job at 857 N. Frontage Road, Wisconsin Dells.
  • Oscar Montoya, Belvidere, Ill., must pay a $2,400 fall-protection citation because the period to contest the violation with a Wisconsin OSHA office has ended. The company was issued the citation Oct. 31 during a roofing job at 1502 Vernon St., Stoughton.
  • Building & Property Maintenance Services LLC, Fond du Lac, reached a formal settlement with OSHA on Jan. 14 to pay a $4,200 disposal-of-waste-materials citation, a $4,200 scaffolding-training citation and two scaffolding citations, totaling $8,400. The company was issued the citations Aug. 18, 2011, during a framing in Fond du Lac.
  • United Contracting Inc., Forest Junction, reached two formal settlements with OSHA on Aug. 26 to reduce seven scaffolding citations, totaling $72,200, to $40,300 and five fall-protection citations, totaling $68,600, to $40,700. The company and OSHA also agreed to dismiss a $4,200 rough-terrain-forklift citation and a $4,200 powered-industrial-trucks citation. The company was issued the citations Nov. 18, 2011, during a highway, street and bridge job and a painting and wall-covering job on two job sites on Highway 41 in Fond du Lac.
