• Kings of the Roof LLC, Milwaukee, must pay a $1,200 eye-and-face-protection citation, a $2,800 fall-protection citation and a $2,800 fall-protection-training citation because the period to contest the violations with a Wisconsin OSHA office has ended. The company was issued the citations April 4 during a roofing job in Janesville.
• Adan Ramirez, Green Bay, must pay a $2,800 safety-training-and-education citation, a $1,600 eye-and-face-protection citation, a $2,800 fall-protection citation and a $2,000 ladders citation because the period to contest the violations with a Wisconsin OSHA office has ended. The company was issued the citations March 7 during a roofing job at 1325 S. Main St., Fond du Lac.
• H&H Utility Excavating Inc., Plymouth, must pay a $3,500 safety-training-and-education citation and a $4,900 electrical-safety-related-work-practices citation because the period to contest the violations with a Wisconsin OSHA office has ended. The company was issued the citations March 12 during a water- and sewer-line job at 2071 Central Drive, Green Bay.
• P.A. McGuire Construction Inc., Highland, reached an informal settlement with OSHA on April 29 to reduce two scaffolding citations, totaling $3,200, to $720. The company was issued the citations April 8 during a masonry job at 1275 Highway 14, Richland Center.
• R&W Trucking Inc., Mukwonago, reached an informal settlement with OSHA on May 1 to reduce a $2,800 requirements-for-excavation-protective-systems citation to $1,960 and a $1,360 ladders citation to $0. The company was issued the citations April 17 during a water- and sewer-line job at S76 W17745 Janesville Road, Muskego.
• Central Painting Inc., Saint John, Ind., reached a formal settlement with OSHA on May 5 to increase two occupational-noise-exposure citations from a total of $0 to $1,540 and to dismiss a $3,080 occupational-noise-exposure citation, a $2,640 gases-vapors-fumes-dusts-and-mists citation and a $3,080 lead citation. The company was issued the citations Sept. 25 during a painting and wall-covering job in Milwaukee.
• Over the Top Roofing & Construction Inc., Menomonee Falls, reached an informal settlement with OSHA on May 6 to reduce a $2,000 aerial-lifts citation to $840. The company was issued the citation April 18 during a siding job at 10916 75th St., Kenosha.
• Premier Building Solutions LLC, Mount Horeb, reached an informal settlement with OSHA on May 7 to reduce a $2,000 scaffolding-training citation to $1,400 and two scaffolding citations, totaling $4,800, to $3,360. The company was issued the citations April 22 during a specialty trade job at W5875 Lintner Road, Wyocena.
• Jurado Construction LLC, Racine, reached an informal settlement with OSHA on May 8 to reduce a $2,000 general-safety-and-health-provisions citation to $800, a $1,200 head-protection citation to $400, a $2,800 scaffolding-training citation to $1,000 and four scaffolding citations, totaling $9,200, to $2,800. The company was issued the citations April 15 during a siding job at 10916 75th St., Kenosha.
• Steve Defere, Sturgeon Bay, reached an informal settlement with OSHA on May 8 to dismiss a $2,800 fall-protection citation, a $2,800 fall-protection-training citation and a $2,800 ladders citation. The company was issued the citations April 8 during a single-family housing job at 5225 Cobblestone Circle, Egg Harbor.
