  • Weatherguard Systems Home Improvements LLC, Appleton, reached a formal settlement with OSHA on July 26, 2013, to reduce a $2,000 guarding-floor-and-wall-openings-and-holes citation to $750 and a $1,200 general-safety-and-health-provisions citation to $0. The company was issued the citations April 30, 2013, during a siding job at 5530 W. Neubert Road, Appleton.
  • Skyview Home Improvements LLC, Altoona, reached an informal settlement with OSHA on June 24 to reduce a $1,600 eye-and-face-protection citation to $800 and a $2,000 fall-protection citation to $1,000. The company was issued the citations June 4 during a roofing job at 4423 Golf Terrace, Eau Claire.
  • Lexington Homes Inc., Green Bay, reached an informal settlement with OSHA on June 25 to reduce a $1,200 aerial-lifts citation to $700. The company was issued the citation June 6 during a commercial and institutional building job at 1900 E. Broadway, Waukesha.
  • Custom Fabricating & Repair Inc., Marshfield, reached an informal settlement with OSHA on June 27 to reduce a $3,000 hazard-communication citation to $0 and two respiratory-protection citations, totaling $8,000, to $0. The company and OSHA also agreed to dismiss two respiratory-protection citations, totaling $18,000. The company was issued the citations June 13 during a building equipment job at 1932 E. 26th St., Marshfield.
  • Badger Underground Inc., Menomonee Falls, reached an informal settlement with OSHA on June 27 to reduce a $2,000 specific-excavation-requirements citation to $1,400 and a $2,000 requirements-for-excavation-protective-systems citation to $1,400. The company was issued the citations May 22 during a water- and sewer-line job at 6221 Lake Drive, Whitefish Bay.
  • Northern Prairie Builders LLC, Burlington, reached an informal settlement with OSHA on June 30 to reduce two scaffolding citations, totaling $3,200, to $2,240. The company was issued the citations June 6 during a framing job at 1416 Serena Lane, Burlington.
  • Applied Flooring Solutions LLC, Green Bay, paid a $2,800 ramp-door citation June 30. The company was issued the citation May 14 during a flooring job at N95 W18273 Country Line Road, Menomonee Falls.
  • Meidam Builders Inc., Appleton, reached an informal settlement with OSHA on July 1 to reduce a $1,600 scaffolding citation to $800, a $2,000 fall-protection citation to $1,000 and a $2,000 ladders citation to $1,000. The company was issued the citations June 6 during a single-family housing job in Grand Chute.
  • A.J. Heinen Inc., New Berlin, reached an informal settlement with OSHA on July 2 to pay a $2,000 lead citation and to reduce a $1,600 cadmium citation to $0 and a $1,600 inorganic arsenic citation to $0. The company was issued the citations June 13 during a site preparation job at 345 W. St. Paul Ave., Milwaukee.
  • Mid Towne Construction Inc., Cross Plains, reached an informal settlement with OSHA on July 3 to reduce a $2,800 fall-protection citation to $1,960 and a $2,000 fall-protection-training citation to $1,400. The company was issued the citations June 20 during a roofing job at 1341 South St., Madison.
  • Mather’s Improvement Service Ltd., Burlington, reached an informal settlement with OSHA on July 3 to reduce a $1,760 scaffolding citation to $1,232 and a $1,760 scaffolding-training citation to $1,232. The company was issued the citations June 27 during a siding job at 365 Pine St., Burlington.
  • Creative Construction of Eau Claire LLC, Fall Creek, reached an informal settlement with OSHA on July 4 to reduce a $2,000 woodworking-tools citation to $1,400. The company was issued the citation June 23 during a commercial and institutional building job at 2836 N. Clairemont Ave., Eau Claire.
  • Precision Builders of Central Wisconsin Inc., Colby, reached an informal settlement with OSHA on July 8 to reduce a $1,600 fall-protection citation to $1,100. The company was issued the citation June 16 during a roofing job at 109 S. Fourth St., Abbotsford.
