• Alberta Vargas-Cruz, Appleton, must pay a $2,640 ladders citation and a $3,520 fall-protection citation because the period to contest the violations with a Wisconsin OSHA office has ended. The company was issued the citations June 19 during a roofing job at 1126 S. 14th St., Manitowoc.

• Oscar Cueto, Appleton, must pay a $1,600 fall-protection citation because the period to contest the violation with a Wisconsin OSHA office has ended. The company was issued the citation June 19 during a roofing job at 947 Ninth St., Neenah.

• Asphalt Contractors Inc., Union Grove, reached a formal settlement with OSHA on July 21 to reduce a $4,900 fall-protection citation to $2,450, a $4,900 stairways-and-ladders-training citation to $0 and a $4,900 ladders citation to $2,450. The company and OSHA also agreed to dismiss a $3,500 general-safety-and-health-provisions citation and to increase a fall-protection-training citation from $3,500 to $4,900. The company was issued the citations March 12 during a specialty trade job at 34215 W. Market St., Burlington.

• Zavet Construction Inc., Otsego, Minn., reached an informal settlement with OSHA on Aug. 4 to reduce a $1,200 general-safety-and-health-provisions citation to $720, a $2,000 fall-protection citation to $1,200 and a $2,000 ladders citation to $1,200. The company was issued the citations July 15 during a framing job at 12214 75th St., Kenosha.

• PRL Excavating Inc., Plymouth, reached an informal settlement with OSHA on Aug. 5 to reduce a $2,800 requirements-for-excavation-protective-systems citation to $1,400 and a $1,200 ladders citation to $600. The company was issued the citations July 16 during a water- and sewer-line job at 85 S. Wacker Drive, Hartford.

• The Roofing Company of Burlington Inc., Burlington, reached an informal settlement with OSHA on Aug. 6 to reduce a $2,640 fall-protection citation to $1,584. The company was issued the citation July 23 during a roofing job at 341 High Ridge Road, Burlington.

• Dan’s Roofing, Kenosha, reached an informal settlement with OSHA on Aug. 6 to reduce a $1,020 fall-protection citation to $612 and a $1,200 fall-protection-training citation to $720. The company was issued the citations July 18 during a roofing job at 1545 15th Court, Kenosha.

• Cassity Construction, Racine, reached an informal settlement with OSHA on Aug. 7 to reduce a $1,200 general-safety-and-health-provisions citation to $840, a $2,800 fall-protection citation to $1,960 and a $1,200 fall-protection-training citation to $0. The company was issued the citations July 17 during a framing job at 8519 Chaucer Circle E., Bristol.

• Steve Yoder, Hillpoint, reached an informal settlement with OSHA on Aug. 7 to reduce a $2,200 ladders citation to $520 and a $6,160 fall-protection citation to $3,080. The company was issued the citations July 17 during a framing job in Verona.

• Leafguard Southeast LLC, Muskego, reached an informal settlement with OSHA on Aug. 8 to reduce a $3,080 woodworking-tools citation to $1,848, a $1,760 scaffolding-training citation to $0 and a $5,280 scaffolding citation to $3,168. The company was issued the citations July 15 during a siding job at 8907 43rd Ave., Kenosha.

• Wenger Construction Inc., Campbellsport, reached an informal settlement with OSHA on Aug. 8 to reduce a $2,000 fall-protection citation to $1,020. The company was issued the citation July 18 during a roofing job at 9322 Loomis Road, Franklin.

• Hector Hernandez, Appleton, reached an informal settlement with OSHA on Aug. 11 to reduce a $1,600 hand-and-power-tools citation to $800. The company was issued the citation July 10 during a roofing job at 2221 Waldo Blvd., Manitowoc.

• Dynamics Construction LLC, Stoughton, reached an informal settlement with OSHA on Aug. 11 to reduce a $2,000 fall-protection citation to $1,400. The company was issued the citation July 17 during a framing job at 610 John Nolen Drive, Madison.

• Holler & Sons Inc., Black Earth, reached an informal settlement with OSHA on Aug. 12 to reduce a $2,800 fall-protection citation to $1,960. The company was issued the citation July 17 during a framing job at 7537 Fallen Oak Drive, Middleton.
