• Custom Made Co., New London, must pay a $2,800 OSHA-Act-of-1970 citation, a $1,600 fall-protection citation and a $1,600 fall-protection-training citation because the period to contest the violations with a Wisconsin OSHA office has ended. The company was issued the citations Jan. 8 during a framing job at W220 N1285 Springdale Road, Pewaukee.

• Mundschau Built Exteriors LLC, Waukesha, must pay a $1,600 fall-protection citation, a $1,600 fall-protection-training citation and a $1,360 ladders citation because the period to contest the violations with a Wisconsin OSHA office has ended. The company was issued the citations Dec. 13 during a siding job at 148 S. Kane St., Burlington.

• Antonio Aguilar Garcia LLC, Clarkesville, Ind., must pay a $2,000 eye-and-face-protection citation, a $2,800 fall-protection citation and a $1,200 ladders citation because the period to contest the violations with a Wisconsin OSHA office has ended. The company was issued the citations Sept. 3 during a roofing job in Manitowoc.

• Vosberg Construction LLC, Columbus, must pay a $15,400 fall-protection citation and a $2,640 eye-and-face-protection citation because the period to contest the violations with a Wisconsin OSHA office has ended. The company was issued the citations Jan. 15 during a framing job in Oregon.

• Lunda Construction Co., Black River Falls, reached a formal settlement with OSHA on Jan. 3 to pay a $7,000 safety-training-and-education citation and two cranes-and-derricks-assembly-disassembly citations, totaling $14,000. The company was issued the citations Oct. 10, 2012, during a highway, street and bridge job on Highway 41 in De Pere.

• August Winter & Sons Inc., Appleton, reached a formal settlement with OSHA on Feb. 10 to dismiss a $2,754 requirements-for-excavation-protective-systems citation. The company was issued the citation Aug. 13 during a plumbing, heating and air conditioning job in Oshkosh.

• Badgerland Metal Building Erectors Inc., Waunakee, reached an informal settlement with OSHA on Feb. 13 to reduce a $2,000 general-safety-and-health-provisions citation to $0 and a $2,800 steel-erection-training citation to $2,000. The company was issued the citations Jan. 24 during a structural steel and precast concrete job at 3275 E. Layton Ave., Cudahy.

• R&R Resource LLC, Wausau, reached an informal settlement with OSHA on Feb. 13 to reduce a $2,800 safety-training-and-education citation to $1,120, a $2,000 lead citation to $800, a $2,800 scaffolding citation to $1,120, a $2,800 fall-protection citation to $1,120 and a $2,800 demolition-preparatory-operations citation to $1,120. The company and OSHA also agreed to dismiss a $2,800 material-handling-equipment citation. The company was issued the citations Jan. 17 during a specialty trade job at 1010 S. Ninth St., Sheboygan.

• R.J. Jurowski Construction Inc., Whitehall, reached an informal settlement with OSHA on Feb. 17 to reduce a $1,607 liquefied-petroleum-gas citation to $1,124.90 and a $2,678 aerial-lifts citation to $1,874.60. The company was issued the citations Feb. 11 during a power- and communication-line job in Arcadia.

• Tek Roofing Co., Eau Claire, reached an informal settlement with OSHA on Feb. 18 to reduce a $1,700 fall-protection citation to $1,020. The company was issued the citation Jan. 23 during a roofing job at 1700 Park Ave., Eau Claire.

• JP Keller Construction LLC, Waunakee, reached an informal settlement with OSHA on Feb. 19 to reduce a $1,760 powered-industrial-trucks citation to $1,232, a $3,080 fall-protection citation to $2,156 and a $3,080 fall-protection-training citation to $2,156. The company was issued the citations Jan. 23 during a framing job in McFarland.
