• Dave Sabotnik, Milwaukee, must pay a $2,040 fall-protection citation because the period to contest the violation has ended. The company was issued the citation Jan. 9 during a roofing job at W254 S427 Pin Oak Lane, Waukesha.

• JLCF Contractors Inc., Muskego, must pay a $3,000 hand-and-power-tools citation and an $8,400 fall-protection citation because the period to contest the violations has ended. The company was issued the citations Jan. 18 during a roofing job at 8731 N. 72nd St., Brown Deer.

• Heartland Homes and Development of the Chippewa Valley Inc., Chippewa Falls, reached an informal settlement with OSHA on Dec. 21 to reduce a $1,530 head-protection citation to $918 and a $2,550 fall-protection citation to $1,530. The company was issued the citations Dec. 2 during a single-family housing job in Eau Claire.

• Skyline Roof-En Co. LLC, Brownsville, reached an informal settlement with OSHA on Jan. 27 to reduce a $1,800 powered-industrial-trucks citation to $900, a $4,200 fall-protection citation to $2,100 and a $4,200 fall-protection-training citation to $0. The company was issued the citations Jan. 6 during a single-family housing job in Mayville.

• CBS Builders Inc., Lodi, reached an informal settlement with OSHA on Feb. 15 to reduce two fall-protection citations, totaling $8,400, to $4,980. The company was issued the citations on Jan. 20 during a single-family housing job at 3700 Parmenter St., Middleton.

• Hellyer Custom Carpentry Inc., Millington, Ill., reached an informal settlement with OSHA on March 2 to reduce a $1,800 powered-industrial-trucks citation to $900, a $2,400 eye-and-face-protection citation to $1,200, a $4,200 fall-protection citation to $2,100 and a $3,000 fall-protection-training citation to $1,500. The company was issued the citations Feb. 14 during a single-family housing job at N646 Swamp Angle Road, Walworth.

• Kos Konstruction Co., Ixonia, reached an informal settlement with OSHA on March 7 to reduce two scaffolding citations, totaling $4,800, to $2,500. The company was issued the citations Feb. 7 during a single-family housing job at W273 N6643 Ardeevin Court, Lisbon.

• Garcia Quality Roofing LLC, Madison, reached an informal settlement with OSHA on March 12 to reduce two fall-protection citations, totaling $8,400, to $1,680. The company was issued the citations Feb. 8 during a single-family housing job at 601 Sayner Court, Madison.

• Oelke Construction Co. Inc., Fall River, reached an informal settlement with OSHA on March 14 to reduce a $4,200 fall-protection citation to $2,100 and a $3,000 fall-protection-training citation to $1,500. The company was issued the citations March 6 during a single-family housing job at N7199 Crystal Lake Road, Beaver Dam.

• Quest Building Corp., Waunakee, reached an informal settlement with OSHA on March 16 to reduce a $3,300 scaffolding citation to $1,000 and a $9,240 fall-protection citation to $3,080. The company was issued the citations Feb. 14 during a single-family housing job at 652 Bear Claw Way, Middleton.

• Luke MacLean Construction, Belleville, reached an informal settlement with OSHA on March 20 to reduce a $4,200 fall-protection citation to $1,680 and a $4,200 fall-protection-training citation to $0. The company was issued the citations Feb. 29 during a single-family housing job at 9321 Spirit St., Madison.

• Trujillo Drywall LLC, Franklin, reached an informal settlement with OSHA on April 13 to dismiss a $1,530 electrical-wiring-design-and-protection citation. The company was issued the citation March 22 during a drywall and insulation job at 9075 Highland Park Ave., Franklin.

• Prahl Roofing & Siding Co., West Bend, reached an informal settlement with OSHA on May 1 to reduce a $2,550 fall-protection citation to $0. The company was issued the citation April 11 during a roofing job at 6044 S. 27th St., Milwaukee.

• Dimensional Contractors LLC, Germantown, reached an informal settlement with OSHA on June 4 to reduce a $3,600 eye-and-face-protection citation to $1,000 and a $4,200 fall-protection citation to $1,400. The company was issued the citations May 10 during a residential remodeling job at 6816 Algonquin Drive, Middleton.

• Terlinden Roofing Inc., Kewaskum, reached an informal settlement with OSHA on June 5 to reduce a $2,800 fall-protection citation to $1,960 and a $1,360 ladders citation to $952. The company was issued the citations May 16 during a roofing job at 4485 Plantation Way, West Bend.

Inc., Wild Rose, reached an informal settlement with OSHA on June 7 to reduce a $1,980 fall-protection citation to $1,300, a $4,620 cranes-and-derricks-power-line-safety-equipment-operations citation to $2,900 and a $3,300 cranes-and-derricks-inspections citation to $0. The company was issued the citations May 14 during a framing job in Fond du Lac.
