• Burg’s Rainguard Services LLC, Kiel, must pay two scaffolding citations, totaling $600, because the period to contest the violations has ended. The company was issued the citations June 30 during a siding job in Kiel.

• Vilchis Roofing, Menasha, must pay a $750 fall-protection citation and a $450 ladders citation because the period to contest the violations has ended. The company was issued the citations June 11 during a roofing job at 12 Greves Court, Appleton.

• Germantown Iron & Steel Corp., Jackson, reached an informal settlement with OSHA on Aug. 10 to reduce two aerial-lifts citations, totaling $5,000, to $2,500. The company was issued the citations Aug. 2 during a structural-steel erection job at 5045 W. Grand Market Drive, Grand Chute.

• Advanced Roofing and Siding Specialists LLC, Green Bay, reached an informal settlement with OSHA on Aug. 12 to reduce a $4,000 fall-protection citation to $2,000. The company was issued the citation Aug. 2 during a roofing job at 1035 Happy Lane, Sheboygan Falls.

• Travis Markiewicz Construction LLC, Oconto Falls, reached an informal settlement with OSHA on Aug. 13 to reduce a $2,000 eye-and-face-protection citation to $600 and to pay an $800 fall-protection citation. The company was issued the citations July 8 during a framing job at 2332 E. Plank Road, Appleton.

• Mr. Builder, Manitowoc, reached an informal settlement with OSHA on Aug. 16 to reduce a $600 fall-protection citation to $420. The company was issued the citation Aug. 4 during a single-family housing job at 3456 Expo Drive, Manitowoc.

• The OCI Group LLC, Stoughton, reached an informal settlement with OSHA on Aug. 20 to reduce a $3,000 fall-protection citation to $1,800 and a $1,200 ladders citation to $720. The company was issued the citations Aug. 6 during a framing job in Verona.

• Nieman Central Wisconsin Roofing Co. Inc., Lyndon Station, reached an informal settlement with OSHA on Aug. 20 to reduce an $800 powered-industrial-trucks citation to $480, an $800 head-protection citation to $480 and a $4,000 fall-protection citation to $2,400. The company was issued the citations Aug. 6 during a roofing job at 105 W. Douglas St., Barneveld.

• Tielens Construction Inc., Luxemburg, paid a $300 fall-protection citation Aug. 20. The company was issued the citation Aug. 9 during a single-family housing job at 6344 Highway 57, Baileys Harbor.

• 3-D Specialty Contracting, Saint Cloud, Minn., reached an informal settlement with OSHA on Aug. 24 to reduce a $750 eye-and-face-protection citation to $375 and a $750 hand-and-power-tools citation to $375. The company was issued the citations Aug. 2 during a finishing job in Ashland.

• Zappa Bros. Excavating Inc., Hudson, reached an informal settlement with OSHA on Aug. 25 to dismiss two specific-excavation-requirements citations, totaling $1,350. The company was issued the citations Aug. 9 during an excavation job at 629 Sixth St. N., Hudson.

• Cornerstone Construction Co. Inc., Pleasant Prairie, reached an informal settlement with OSHA on Aug. 26 to reduce a $1,050 fall-protection citation to $500 and a $1,050 fall-protection-training citation to $0. The company and OSHA also agreed to dismiss a $1,050 fall-protection citation and a $1,050 fall-protection-training citation. The company was issued the citations Aug. 10 during a framing job at 7308 60th St., Kenosha.

• Berg’s Bobcat Service, Colfax, reached an informal settlement with OSHA on Aug. 26 to reduce a $750 safety-training-and-education citation to $525, a $750 head-protection citation to $525 and a $750 requirements-for-excavation-protective-systems citation to $525. The company was issued the citations Aug. 9 during a water- and sewer-line job at E202 Highway E Menomonie St., Elk Mound.

• Altmann Construction Co. Inc., Wisconsin Rapids, reached an informal settlement with OSHA on Aug. 26 to reduce a $1,575 self-propelled-elevating-work-platform citation to $1,000 and a $900 electrical-wiring-methods citation to $0. The company was issued the citations Aug. 2 during a nonresidential building job in Appleton.

• Karl Kimmes Well & Pump Inc., Two Rivers, reached an informal settlement with OSHA on Aug. 30 to reduce a $1,500 specific-excavation-requirements citation to $1,000. The company and OSHA also agreed to dismiss a $1,500 eye-and-face-protection citation and a $600 electrical-wiring-design-and-protection citation. The company was issued the citations Aug. 4 during a water well drilling job in Manitowoc.

• Operation Fresh Start Inc., Madison, reached an informal settlement with OSHA on Aug. 31 to reduce a $900 ladders citation to $500 and three scaffolding citations, totaling $15,000, to $7,000. The company and OSHA also agreed to dismiss two scaffolding citations, totaling $12,000. The company was issued the citations Aug. 9 during a residential remodeling job at 5028 Camden Road, Madison.

• Park Avenue Development Inc., Grafton, reached an informal settlement with OSHA on Sept. 2 to reduce a $600 fall-protection citation to $420 and a $600 fall-protection-training citation to $420. The company was issued the citations June 17 during a roofing job at 748 Greig Trail, Stoughton.

• Flatwater Construction Inc., Oak Brook, Ill., reached an informal settlement with OSHA on Sept. 3 to reduce a $600 scaffolding citation to $420 and a $750 fall-protection citation to $525. The company was issued the citations Aug. 9 during a roofing job at 916 Lake Shore Drive W., Ashland.

• Mike Blindauer Sheet Metal & Roofing Inc., Green Bay, paid a $1,600 fall-protection citation Sept. 7. The company was issued the citation July 28 during a roofing job at 825 Potts Ave., Green Bay.

• Integrity Environmental Services Inc., Waukesha, reached a formal settlement with OSHA on Sept. 10 to dismiss a $750 fall-protection citation. The company was issued the citation May 12 during a remediation job in Milwaukee.
