• T.D. Countertops LLC, Portage, must pay a $1,200 stairways citation because the period to contest the violation with a Wisconsin OSHA office has ended. The company was issued the citation Dec. 17 during a tile and terrazzo job at 14901 W. Library Lane, New Berlin.
• Ballweg Construction Inc., Cross Plains, reached a formal settlement with OSHA on March 19 to reduce a $2,000 head-protection citation to $0, a $2,800 fall-protection citation to $1,120, a $2,800 truss citation to $1,120 and a $2,800 scissor-lift citation to $1,120. The company was issued the citations Nov. 16 during a framing job at 424 Highway 14, Arena.
• Knoelke Construction LLC, Oconomowoc, reached an informal settlement with OSHA on April 17 to pay a $4,000 scaffolding citation and to reduce a $1,600 head-protection citation to $720, a $4,000 fall-protection citation to $0 and a $4,000 fall-protection-training citation to $2,000. The company was issued the citations March 25 during a framing job at 1426 Deer Path, Waukesha.
• Fred Hall Construction LLC, Viola, reached an informal settlement with OSHA on April 17 to reduce a $3,080 fall-protection citation to $2,156 and a $2,200 fall-protection-training citation to $1,540. The company was issued the citations March 26 during a framing job at 405 Lucerne Drive, Verona.
• Mayo Clinic Health Systems, Menomonie, reached an informal settlement with OSHA on April 18 to reduce a $3,000 electrical-wiring-methods citation to $1,500. The company was issued the citation March 27 during a commercial and institutional building job at 2321 Stout Road, Menomonie.
• U Builders LLC, Lodi, reached a formal settlement with OSHA on April 22 to reduce a $3,080 fall-protection citation to $1,000 and a $2,200 fall-protection-training citation to $700. The company was issued the citations Jan. 10 during a framing job at 152 West End Circle, Verona.
• VDH Electric Inc., Green Bay, reached an informal settlement with OSHA on April 22 to reduce a $2,380 electrical-wiring-methods citation to $500. The company was issued the citation March 28 during an electrical job at 433 Main St., Green Bay.
• Steve Wilder, Tomah, reached an informal settlement with OSHA on April 22 to reduce a $2,400 fall-protection citation to $1,200 and a $1,600 fall-protection-training citation to $800. The company was issued the citations March 28 during a roofing job at 1654 S. 11th Ave., Fort McCoy.
• Steve Fischer Construction LLC, Sobieski, reached an informal settlement with OSHA on April 24 to reduce a $1,320 ladders citation to $0 and a $4,400 fall-protection citation to $3,000. The company was issued the citations March 26 during a framing job in Appleton.
