• Best in Homes LLC, Beloit, must pay a $2,800 head-protection citation, a $2,800 fall-protection citation and a $2,800 ladders citation because the period to contest the violations with a Wisconsin OSHA office has ended. The company was issued the citations Sept. 26 during a residential remodeling job in Madison.
• Travis Markiewicz Construction LLC, Oconto Falls, must pay a $2,000 general-safety-and-health-provisions citation and a $2,000 fall-protection citation because the period to contest the violations with a Wisconsin OSHA office has ended. The company was issued the citations Aug. 7 during a multifamily housing job at 1821 Meadow Lane, Waukesha.
• Lopez Construction LLC, Madison, must pay a $400 abatement verification citation because the period to contest the violation with a Wisconsin OSHA office has ended. The company was issued the citation Aug. 25 in connection with a siding job on Pinehurst Drive in Verona.
• Madison Exteriors Roofing & Siding, Madison, must pay a $1,360 eye-and-face-protection citation and a $2,800 fall-protection citation because the period to contest the violations with a Wisconsin OSHA office has ended. The company was issued the citations Sept. 8 during a roofing job at 518 Borg Road, Delavan.
• All Gutters LLC, Portage, must pay a $2,400 fall-protection citation and a $2,400 ladders citation because the period to contest the violations with a Wisconsin OSHA office has ended. The company was issued the citations Sept. 5 during a roofing job at 215 Jennifer Lane, Fall River.
• Above the Peak Construction LLC, Beloit, must pay a $440 abatement-verification citation because the period to contest the violation with a Wisconsin OSHA office has ended. The company was issued the citation Aug. 14 in connection with a roofing job at 2355 Granite Terrace, Beloit.
• Nick’s Welding & Fabricating Inc., Hixton, reached an informal settlement with OSHA on Nov. 4 to reduce a $1,320 respiratory-protection citation to $792 and a $1,320 electrical-wiring-design-and-protection citation to $792. The company was issued the citations Oct. 16 during a specialty trade job at N8847 Prospect Road, Hixton.
• Vince Parr, Milwaukee, reached an informal settlement with OSHA on Nov. 5 to reduce a $2,000 general-safety-and-health-provisions citation to $1,000, a $2,800 fall-protection citation to $1,400 and a $2,000 ladders citation to $0. The company was issued the citations Oct. 16 during a roofing job at S105 W20676 Valerie Drive, Muskego.
• SJS Construction Inc., Oak Creek, reached an informal settlement with OSHA on Nov. 6 to reduce a $1,600 eye-and-face-protection citation to $1,000, a $2,000 ladders citation to $0, a $2,800 fall-protection citation to $0 and two scaffolding citations, totaling $4,000, to $3,000. The company was issued the citations Oct. 16 during a roofing job at 516 Arbor Oaks Lane, Waukesha.
• Concrete & Masonry Restoration LLC, Milwaukee, reached an informal settlement with OSHA on Nov. 7 to pay a $3,850 respiratory-protection citation and to reduce a $3,850 gases-vapors-fumes-dusts-and-mists citation to $0. The company was issued the citations Oct. 17 during a poured concrete foundation and structure job at 214 N. Carroll St., Madison.
• Mavo Systems Inc., White Bear Lake, Minn., reached an informal settlement with OSHA on Nov. 10 to reduce a $6,000 asbestos citation to $4,200. The company was issued the citation Oct. 31 during a remediation job at 124 Graham Ave., Eau Claire.
