• Avila’s Construction LLC, Milwaukee, must pay a $750 fall-protection citation, a $750 fall-protection-training citation and a $600 ladders citation because the period to contest the violations has ended. The company was issued the citations July 29 during a nonresidential building job at 14835 W. Lisbon Road, Brookfield.

• Cuellar Construction, West Bend, must pay a $1,000 fall-protection citation because the period to contest the violation has ended. The company was issued the citation July 29 during a roofing job in Mequon.

• Urban Construction Administration Inc., Madison, must pay a $2,000 fall-protection citation because the period to contest the violation has ended. The company was issued the citation July 8 during a masonry job in Madison.

• Amigo Construction LLC, Madison, must pay a $1,050 eye-and-face-protection citation, a $1,500 fall-protection citation and a $1,500 ladders citation because the period to contest the violations has ended. The company was issued the citations July 13 during a roofing job at 215 Madison St., Beaver Dam.

• Cesar Mares, Johnson Creek, must pay a $1,500 fall-protection citation, a $1,500 fall-protection-training citation, a $450 stairways-and-ladders-training citation and two ladders citations, totaling $900, because the period to contest the violations has ended. The company was issued the citations July 16 during a roofing job at 9225 Crosswinds Lane, Madison.

• 24 Karat Construction, Beaver Dam, must pay a $750 fall-protection citation and a $750 fall-protection-training citation because the period to contest the violations has ended. The company was issued the citations July 8 during a roofing job at 215 Fourth St., Beaver Dam.

• Perfect Exteriors Inc., New Berlin, reached an informal settlement with OSHA on Aug. 18 to reduce a $450 head-protection citation to $0, a $750 scaffolding citation to $500 and a $750 scaffolding-training citation to $0. The company was issued the citations July 29 during a roofing job at 18265 San Lucas Court, Brookfield.

• LP Roofing LLC, Eau Claire, agreed to a stipulation and settlement Aug. 20 to reduce a $1,600 eye-and-face-protection citation to $0 and two scaffolding citations, totaling $1,600, to $0. The company was issued the citations June 10 during a roofing job at 1811 Babcock St., Eau Claire.

• Rios Roofing LLC, Menasha, reached an informal settlement with OSHA on Aug. 31 to reduce a $600 fall-protection citation to $420. The company was issued the citation Aug. 24 during a roofing job at 517 Gray St., Green Bay.

• Wittman Construction LLC, Appleton, reached an informal settlement with OSHA on Sept. 8 to reduce a $600 safety-training-and-education citation to $300. The company was issued the citation Aug. 24 during a wrecking and demolition job at 2004 Casaloma Drive, Appleton.

• Cream City Awning & Patio Inc., Kewaskum, reached an informal settlement with OSHA on Sept. 8 to reduce a $600 eye-and-face-protection citation to $360, a $750 fall-protection citation to $450 and three ladders citations, totaling $2,250, to $890. The company and OSHA also agreed to dismiss two ladders citations, totaling $1,500. The company was issued the citations Aug. 25 during a specialty trade job at 7420 W. Good Hope Road, Milwaukee.

• Gregg Laffin Builders LLC, Ixonia, reached an informal settlement with OSHA on Sept. 9 to reduce a $375 rough-terrain-forklift citation to $250, a $375 fall-protection-training citation to $0, a $375 scaffolding citation to $250 and a $375 fall-protection citation to $0. The company and OSHA also agreed to dismiss a $375 scaffolding citation and a $375 fall-protection citation. The company was issued the citations Aug. 23 during a single-family housing job at N37 W23820 Broken Hill Circle, Pewaukee.

• Chippewa Concrete Services Inc., Chippewa Falls, reached an informal settlement with OSHA on Sept. 10 to reduce a $1,250 fall-protection citation to $875 and a $1,250 ladders citation to $875. The company was issued the citations Aug. 25 during a concrete job in Rice Lake.

• Albin Construction Inc., Wind Lake, reached an informal settlement with OSHA on Sept. 15 to reduce an $800 fall-protection citation to $400 and an $800 ladders citation to $400. The company was issued the citations Aug. 25 during a masonry job at 6001 60th Ave., Kenosha.

• Gibbs Home Improvement Inc., Glendale, reached an informal settlement with OSHA on Sept. 15 to reduce a $750 fall-protection citation to $300. The company was issued the citation Aug. 25 during a roofing job at 5154 N. Idlewild Ave., Whitefish Bay.

• JJC Premium Roofing & Sheet Metal LLC, Milwaukee, reached an informal settlement with OSHA on Sept. 16 to reduce a $600 head-protection citation to $400 and a $1,500 fall-protection citation to $800. The company and OSHA also agreed to dismiss a $750 ladders citation. The company was issued the citations Aug. 26 during a roofing job at 3745 N. Booth St., Milwaukee.

• Raymond Quality Services Inc., Franklin, reached an informal settlement with OSHA on Sept. 16 to pay a $750 fall-protection citation and to reduce a $750 ladders citation to $0 and a $750 fall-protection-training citation to $0. The company and OSHA also agreed to dismiss a $750 fall-protection citation and a $750 fall-protection-training citation. The company was issued the citations Aug. 26 during a roofing job at 3873 S. 75th St., West Allis.
