• Adair and Son Sewer and Water Inc., Milwaukee, must pay a $2,800 requirements-for-excavation-protective-systems citation because the period to contest the violations with a Wisconsin OSHA office has ended. The company was issued the citation Jan. 23 during a water- and sewer-line job at 5220 S. 76th St., Greendale.

• Kuhn’s Home Improvements LLC, West Allis, must pay a $5,600 fall-protection citation and a $3,200 ladders citation because the period to contest the violations with a Wisconsin OSHA office has ended. The company was issued the citations Jan. 23 during a roofing job at 2717 S. Herman St., Milwaukee.

• Galindo Roofing, Randolph, must pay a $2,800 fall-protection citation and a $2,800 fall-protection-training citation because the period to contest the violations with a Wisconsin OSHA office has ended. • Choice Construction Companies Inc., Menomonee Falls, reached an informal settlement with OSHA on Jan. 25 to pay a $4,410 multiple-crane-derrick-lifts-supplemental-requirements citation and to reduce a $4,410 general-safety-and-health-provisions citation to $3,590 and a $4,410 cranes-and-derricks-training citation to $0. The company was issued the citations Dec. 27 during a highway, street and bridge job on Highway 41 in Oshkosh.

• Weather Safe Construction Inc., Green Bay, reached an informal settlement with OSHA on March 5 to reduce a $2,800 fall-protection citation to $1,500. The company was issued the citation Feb. 11 during a roofing job at 4725 Sylvan St., Howard.

• Thomas Industrial Coatings Inc., Pevely, Mo., reached an informal settlement with OSHA on March 12 to reduce three lead citations, totaling $13,770, to $3,672. The company was issued the citations Feb. 19 during a painting and wall-covering job in Highway 89 in Lake Mills.

• Walt’s Petroleum Service Inc., Schofield, reached an informal settlement with OSHA on March 12 to reduce a $4,900 specific-excavation-requirements citation to $3,000 and a $4,900 requirements-for-excavation-protective-systems citation to $3,000. The company was issued the citations Feb. 13 during a water- and sewer-line job at 1129 S. Park St., Madison.

• Holtz Lime Gravel & Excavating Inc., Loganville, reached an informal settlement with OSHA on March 12 to reduce a $1,600 specific-excavation-requirements citation to $960, a $1,600 requirements-for-excavation-protective-systems citation to $0 and a $1,200 ladders citation to $720. The company was issued the citations Feb. 25 during a water- and sewer-line job at 210 Highway 154, Loganville.
