• Acme Solutions Roofing LLC, Appleton, must pay a $2,800 fall-protection-training citation, a $4,000 eye-and-face-protection citation and a $5,600 fall-protection citation because the period to contest the violations with a Wisconsin OSHA office has ended. The company was issued the citations Nov. 1 during a roofing job at 1531 Prospect Ave., Appleton.
• Southern Wisconsin Roofing Co. Inc., Madison, reached an informal settlement with OSHA on Nov. 30 to reduce a $5,600 fall-protection citation to $2,800. The company was issued the citation Nov. 6 during a roofing job at 1631 Metalcrest Ave., Middleton.
• Design Structures LLC, Middleton, reached an informal settlement with OSHA on Dec. 4 to reduce a $2,800 fall-protection-training citation to $0 and two fall-protection citations, totaling $4,400, to $2,800. The company was issued the citations Nov. 8 during a commercial and institutional building job at 8401 Durand Ave., Sturtevant.
• American Demolition Corp., Elgin, Ill., reached an informal settlement with OSHA on Dec. 6 to reduce a $2,100 hazard-communication citation to $1,800 and a $2,100 eye-and-face-protection citation to $1,200. The company was issued the citations Nov. 16 during a site preparation job at 800 Langdon St., Madison. American Demolition also reached an informal settlement with OSHA on Dec. 6 to reduce a $5,390 fire-prevention citation to $2,700. The company was issued the citation Dec. 3 during the project on Langdon.
• Quality Erectors & Sheeters Inc., Dane, reached an informal settlement with OSHA on Dec. 10 to reduce a $1,600 rough-terrain-forklift citation to $1,000 and a $2,000 concrete-and-masonry citation to $1,200. The company was issued the citations Nov. 14 during a structural steel and precast concrete job at 5979 Endres Road, Waunakee.
