• OLK Construction Inc., Hortonville, must pay two aerial-lifts citations, totaling $9,240, because the period to contest the violations with a Wisconsin OSHA office has ended. The company was issued the citations Sept. 16 during a structural steel and precast concrete job at 375 Buffalo St., Sheboygan Falls.

• W.W. Timbers Inc., Chicago Ridge, Ill., reached a formal settlement with OSHA on Aug. 7 to reduce a $2,040 general-safety-and-health-provisions citation to $1,155, a $1,700 aerial-lifts citation to $1,155 and a $2,040 fall-protection citation to $1,155. The company was issued the citations March 20 during a foundation, structure and building exterior job at 1070 224th Ave., Kansasville.

• Kickapoo Homes Inc., Viroqua, reached an informal settlement with OSHA on Nov. 5 to reduce a $2,400 fall-protection citation to $0, a $2,400 ladders citation to $1,200 and three scaffolding citations, totaling $8,400, to $4,200. The company was issued the citations Oct. 15 during a single-family housing job at S5277 County Road SS, Viola.
