• Gonzalez Construction LLC, Chicago, must pay a $750 fall-protection citation because the period to contest the violation has ended. The company was issued the citation June 3 during a roofing job in Racine.

• Hill Construction, Edgar, reached a formal settlement with OSHA on April 9 to reduce a $1,050 head-protection citation, a $1,500 eye-and-face-protection citation, a $1,500 woodworking-tools citation, a $750 electrical-wiring-methods citation, a $1,500 fall-protection citation and a $750 ladders citation to a total of $2,460. The company was issued the citations Dec. 11 during a single-family housing job at 14672 Second Ave., Ringle.

• Mark Gajeski Construction Inc., Pulaski, reached a formal settlement with OSHA on June 7 to reduce a $1,600 fall-protection citation to $400 and a $400 fall-protection-training citation to $0. The company was issued the citations July 7, 2009, during a single-family housing job at 2017 Morning Dew Lane, De Pere.

• Vanco Construction Inc., Seymour, reached an informal settlement with OSHA on July 20 to reduce a $1,500 requirements-for-excavation-protective-systems citation to $1,065 and a $450 specific-excavation-requirements citation to $100. The company was issued the citations July 8 during a water- and sewer-line job in Little Chute.

• Hau & Sippel Builders Inc., Glenbeulah, reached an informal settlement with OSHA on July 21 to reduce a $525 forklift citation to $367.50. The company was issued the citation July 8 during a framing job in Plymouth.

• Nek Excavating Inc., Pleasant Prairie, reached an informal settlement with OSHA on July 21 to reduce three specific-excavation-requirements citations, totaling $2,250, to $750. The company was issued the citations July 9 during a water- and sewer-line job at 8503 94th Ave., Pleasant Prairie.

• RT Exterior Inc., Schaumburg, Ill., reached an informal settlement with OSHA on July 22 to reduce a $1,500 scaffolding citation to $750 and a $1,500 ladders citation to $750. The company was issued the citations July 8 during a siding job at 8383 94th Ave., Pleasant Prairie.

• Phase One Carpenter Contractors Inc., New Berlin, paid a $375 scaffolding citation July 23. The company was issued the citation July 6 during a framing job at 12225 Peregrine Lane, Brookfield.

• Van Offeren Electric LLC, Appleton, reached an informal settlement with OSHA on July 23 to reduce a $300 eye-and-face-protection citation to $210 and a $300 fall-protection citation to $210. The company was issued the citations July 9 during an electrical job in Oshkosh.

• John Koss Builders Inc., Manitowoc, reached an informal settlement with OSHA on July 28 to reduce three scaffolding citations, totaling $2,400, to $1,200. The company was issued the citations July 8 during a framing job at 1300 New York Ave., Manitowoc.

• Jossart Brothers Inc., De Pere, reached an informal settlement with OSHA on July 29 to reduce a $1,500 specific-excavation-requirements citation to $1,050, a $1,500 requirements-for-excavation-protective-systems citation to $1,050 and a $750 ladders citation to $525. The company was issued the citations July 8 during an excavation job at 1132 E. Wisconsin Ave., Appleton.

• Triad Builders LLC, Brooklyn, paid a $4,000 fall-protection citation Aug. 4. The company was issued the citation July 9 during a finishing job at 3326 Walter Way, Green Bay.

• K&M Homes LLC, La Valle, reached an informal settlement with OSHA on Aug. 5 to reduce a $1,050 eye-and-face-protection citation to $682.50, a $600 woodworking-tools citation to $390 and a $1,050 fall-protection citation to $682.50. The company was issued the citations July 8 during a framing job in Black River Falls.

• Knoelke Construction LLC, Oconomowoc, reached a formal settlement with OSHA on Aug. 6 to reduce a $1,500 fall-protection citation to $900, a $1,500 fall-protection-training citation to $0, a $600 eye-and-face-protection citation to $360, a $375 stairways citation to $225 and two scaffolding citations, totaling $3,000, to $900. The company and OSHA also agreed to dismiss a $1,500 fall-protection citation, a $1,500 fall-protection-training citation and two scaffolding citations, totaling $3,000. The company was issued the citations April 30 during a framing job at 119 Jersey Circle, Waukesha.
