• Bulldog Construction LLC, Stoughton, must pay a $2,200 fall-protection- , a $15,400 fall-protection citation, a $4,400 ladders citation and a $440 -verification citation because the period to contest the violations with a Wisconsin OSHA office has ended. The company received the abatement-verification citation Aug. 21 and the other three May 31 during and following a roofing job in . Bulldog also must pay a $3,080 fall-protection-training citation, a $15,400 fall-protection citation and a $440 abatement-verification citation because the contest period has ended. The company received the abatement-verification citation Sept. 19 and the other two July 30 during and following a job in Sun Prairie.

• Troy Hojnacki, Stevens Point, must pay a $2,800 aerial-lifts citation because the period to contest the violation with a Wisconsin OSHA office has ended. The company was issued the citation July 15 during a painting and wall covering job at 912 Main St., Stevens Point.

• Panas Roofing & Siding, Delavan, must pay a $1,600 electrical-installation--requirements citation and a $1,200 ladders citation because the period to contest the violations with a Wisconsin OSHA office has ended. The company was issued the citations Aug. 13 during a siding job in Green Bay.

• MJ’Z Contractors LLC, Neenah, must pay a $1,200 eye-and-face-protection citation, a $2,000 fall-protection citation and a $2,000 ladders citation because the period to contest the violations with a Wisconsin OSHA office has ended. The company was issued the citations Aug. 13 during a roofing job at 112 E. Taft Ave., Appleton. MJ’Z also must pay a $2,800 fall-protection citation and a $1,600 ladders citation because the contest period has ended. The company was issued the citations Aug. 14 during a roofing job at 534 S. 32nd St., Manitowoc.

• Last Drop Roofing, Siding & Seamless Gutters LLC, Two Rivers, must pay a $1,760 fall-protection citation because the period to contest the violation with a Wisconsin OSHA office has ended. The company was issued the citation Aug. 20 during a roofing job in Manitowoc.

• Rogelio Gonzalez-Flores, Wellford, S.C., must pay a $2,800 fall-protection citation, a $2,000 ladders citation and a $2,800 fall-protection-training citation because the period to contest the violations with a Wisconsin OSHA office has ended. The company was issued the citations July 31 during a roofing job at 1231 S. Seventh St., Manitowoc.

• Moises Martinez, Appleton, must pay a $2,000 eye-and-face-protection citation and a $2,800 fall-protection citation because the period to contest the violations with a Wisconsin OSHA office has ended. The company was issued the citations July 11 during a roofing job at 332 S. Locust St., Appleton.

• Luis Ramiro Morales, Appleton, must pay a $2,000 fall-protection citation because the period to contest the violation with a Wisconsin OSHA office has ended. The company was issued the citation June 27 during a roofing job in Appleton.

• C&E Wurzer Builders LLC, Chippewa Falls, must pay a $1,200 hazard-communication citation, a $2,800 fall-protection-training citation, a $1,600 stairways citation and two fall-protection citations, totaling $9,800, because the period to contest the violations with a Wisconsin OSHA office has ended. The company was issued the citations Sept. 11 during a framing job at 241 Wilson Ave., Altoona.

• Matafied Exterior and Interior, Kimberly, must pay a $2,000 fall-protection citation and a $2,000 ladders citation because the period to contest the violations with a Wisconsin OSHA office has ended. The company was issued the citations Aug. 29 during a roofing job at 3200 N. Richmond St., Appleton.

• Hooper Corp., Madison, reached a formal settlement with OSHA on June 7 to dismiss two requirements-for-excavation-protective-systems citations, totaling $9,350. The company was issued the citations June 25, 2012, during a plumbing, heating and air conditioning job at 2601 Agriculture Drive, Madison.

• PCI Roads LLC, St. Michael, Minn., reached an informal settlement with OSHA on Oct. 22 to reduce a $5,000 respiratory-protection citation to $0. The company was issued the citation Sept. 26 during a highway, street and bridge job in Madison.

• Haas Sons Inc., Thorp, reached an informal settlement with OSHA on Oct. 22 to reduce a $12,600 ladders citation to $8,190. The company was issued the citation Sept. 12 during a water- and sewer-line job at 1755 Second St., .

• Cliff Bergin & Associates Inc., , reached an informal settlement with OSHA on Oct. 29 to reduce a $4,900 electrical-wiring-methods citation to $2,450 and three criteria-for-personal-protective-equipment citations, totaling $11,900, to $2,450. The company was issued the citations Sept. 26 during a plumbing, heating and air conditioning job at 6300 Donges Bay Road, Mequon.

• Fernando Carrillo, Mansfield, Texas, reached an informal settlement with OSHA on Oct. 29 to reduce a $2,000 fall-protection citation to $1,000, a $1,600 safety-training-and-education citation to $800, a $2,400 eye-and-face-protection citation to $1,200 and a $1,600 ladders citation to $650. The company was issued the citations Aug. 23 during a roofing job at 1513 Waldo Blvd., Manitowoc.

• Wiczek’s Top Notch Construction LLC, Greenfield, reached an informal settlement with OSHA on Oct. 30 to reduce two fall-protection citations, totaling $8,360, to $3,300. The company was issued the citations Sept. 25 during a roofing job at 2124 S. 76th St., West Allis.

• Balyasny Asset Management LP, Milwaukee, reached an informal settlement with OSHA on Nov. 1 to reduce two stairways citations, totaling $5,400, to $3,800. The company was issued the citations Sept. 23 during a residential remodeling job at 10633 W. Vista View Court, Mequon.

• JI Roofing LLC, Manitowoc, reached an informal settlement with OSHA on Nov. 6 to reduce a $2,000 -chutes citation to $1,400, a $2,800 eye-and-face-protection citation to $1,960 and two asbestos citations, totaling $5,600, to $0. The company and OSHA also agreed to dismiss a $2,800 asbestos citation. The company was issued the citations Oct. 7 during a roofing job at 7530 St. Pat’s Road, Reedsville.
