• Zastrow Construction LLC, North Freedom, must pay a $400 abatement-verification citation because the period to contest the violation with a Wisconsin OSHA office has ended. The company was issued the citation March 6 in connection with a roofing job in North Freedom.
• Bolden Brothers Masonry, Sparta, must pay a $1,600 scaffolding citation because the period to contest the violation with a Wisconsin OSHA office has ended. The company was issued the citation Feb. 12 during a masonry job at N9598 Highway 54, Black River Falls.
• JJC Premium Roofing & Sheet Metal LLC, Menomonee Falls, must pay a $14,000 fall-protection citation following a review commission ruling Oct. 25. The company was issued the citation Feb. 5, 2013, during a roofing job at 2406 E. Oklahoma Ave., Milwaukee.
• Ice Construction LLC, Oshkosh, reached a formal settlement with OSHA on April 7 to dismiss a $1,600 fall-protection citation and a $1,600 ladders citation. The company was issued the citations Oct. 23 during a roofing job at 1268 S. 32nd St., Manitowoc.
• Valdex Construction Corp., Niles, Ill., reached an informal settlement with OSHA on April 11 to reduce a $1,600 fall-protection citation to $1,120 and two ladders citations, totaling $2,800, to $1,960. The company was issued the citations March 21 during a roofing job at 1070 224th Ave., Kansasville.
• Hagen Contractors LLC, Sturtevant, reached an informal settlement with OSHA on April 16 to reduce a $2,400 general-safety-and-health-provisions citation to $1,200, a $1,700 electrical-safety-related-work-practices citation to $0, a $2,400 fall-protection citation to $0 and two scaffolding citations, totaling $4,800, to $1,200. The company was issued the citations April 1 during a framing job in Burlington.
• Leafguard Southeast LLC, Muskego, reached an informal settlement with OSHA on April 17 to reduce a $2,800 general-safety-and-health-provisions citation to $1,000, a $2,800 scaffolding citation to $2,000 and a $2,800 fall-protection-training citation to $2,000. The company was issued the citations March 28 during a siding job at 980 Elm Road, Cedarburg.
• Lopez Construction LLC, Madison, reached an informal settlement with OSHA on April 21 to reduce a $2,000 safety-training-and-education citation to $1,000; a $2,800 fall-protection citation to $1,400; a $2,000 stairways citation to $1,000; two ladders citations, totaling $4,000, to $2,000; and two scaffolding citations, totaling $4,800, to $2,500. The company was issued the citations March 26 during a siding job in Verona.
• Lutz Inc., Madison, reached an informal settlement with OSHA on April 21 to reduce a $2,000 stairways-and-ladders-training citation to $1,400. The company was issued the citation April 11 during a specialty trade job at 439 Jones St., Cambridge.
• Kruczek Construction Inc., Green Bay, reached an informal settlement with OSHA on April 25 to reduce a $2,000 specific-excavation-requirements citation to $1,400 and a $1,200 requirements-for-excavation-protective-systems citation to $840. The company was issued the citations April 2 during a water- and sewer-line job on North Overland Road in Hobart.
