• John Wanta Builders Inc., Hatley, must pay a $2,000 aerial-lifts and a $2,000 steel-erection-training citation because the period to contest the violations with a Wisconsin OSHA office has ended. The company was issued the citations March 28 during a structural steel and job on Highway T in Withee.

• Above the Peak Construction LLC, Beloit, reached a formal settlement with OSHA on Feb. 15 to reduce a $2,800 fall-protection citation to $600 and a $2,800 fall-protection-training citation to $600. The company was issued the citations July 11 during a roofing job at 1961 Church St., Beloit.

• Encompass Construction LLC, , reached a formal settlement with OSHA on March 8 to reduce a $3,000 fall-protection citation to $750. The company was issued the citation Feb. 8, 2012, during a single-family housing job at 1141 Greenway Blvd., Columbus.

• J&J Construction, Madison, reached a formal settlement with OSHA on April 4 to dismiss a $7,920 fall-protection citation and a $7,920 fall-protection-training citation. The company was issued the citations Aug. 29, 2011, during a roofing job on Oxford Lane in Cambridge.

• Aniceto Rodriguez, Saint Charles, Mo., reached an informal settlement with OSHA on April 19 to reduce a $2,800 fall-protection citation to $1,800. The company was issued the citation Feb. 5 during a roofing job on Tonyawartha Trail in Monona.

• LP Customs LLC, DeForest, reached an informal settlement with OSHA on April 25 to reduce a $2,800 scaffolding citation to $1,200, a $2,800 scaffolding-training citation to $0 and a $2,800 ladders citation to $1,200. The company was issued the citations April 1 during a residential remodeling job at 917 Lindsey Court, Cottage Grove.

• Gateway Concrete Forming Systems Inc., Mequon, reached an informal settlement with OSHA on April 25 to reduce a $3,850 fall-protection citation to $1,925. The company was issued the citation April 11 during a poured concrete foundation and structure job at 333 Pine Ridge Blvd., Wausau.

• Michiels Countryside Construction Inc., Denmark, reached an informal settlement with OSHA on April 25 to reduce a $1,200 ladders citation to $700 and two scaffolding citations, totaling $2,800, to $1,400. The company was issued the citations April 3 during a single-family housing job at 5803 Bluffledge Road, Sturgeon Bay.
